Responsibilities

The Nurse Practitioner delivers annual risk assessment in a residential setting or telehealth, within the scope of practice for a Nurse Practitioner, as delegated by the Collaborative Physician.

Essential Duties and Responsibilities

Conduct comprehensive in-home health risk assessments to identify all active and chronic disease conditions, as well as determine all physical, mental, and social needs present at the time of the visit
Takes history, examines, determines diagnoses.
Provides written documentation of patient visit, per NCQA standards
Takes patient vital signs, as necessary.
Places case management referrals and communicates with PCP as necessary.
Communicates with patients, caregivers, agency nurses, other providers and vendors as necessary to assure proper diagnosis.
Performs all clinical duties while observing OSHA Universal Precautions
Maintains patient confidentiality
Attends required meetings and in-services and participates in committees, as requested
Participates in professional development activities and maintains professional licenses and affiliations

Active/unrestricted nurse practitioner license to practice in coverage states

The Advanced Practice Provider will be responsible for comprehensive direct patient care management and will manage a caseload of patients by exercising autonomous decision making in the assessment, diagnosis, and initiation of delegated medical treatments. The Inpatient Oncology APP Team is an APP-staffed service with the support of a Hematology/Oncology Attending. The team rounds in the morning and the APPs present patients with their assessments and plans. APPs then carry out the plan for the day, calling consults, placing orders, admitting expected and unexpected admits. This candidate will be trained in the management of both hematologic and solid tumor malignancies, as well as oncologic emergencies (eg neutropenic fever, tumor lysis syndrome, cytokine release syndrome, neurotoxicity due to therapeutic agents, DIC, HLH, sepsis, and cord compression).
