Position Summary:

Provides clinical knowledge and expertise in coordinating patient care in the Grace Cottage Family Health setting as directed by the medical provider. Collaborate in team-based care within a Patient-Centered Medical Home (PCMH) to deliver coordinated, comprehensive, and personalized care that supports patient wellness and improve health outcomes. This position will primarily be responsible for pool management for the providers, answering phone calls, replying to portal messages, and back-up for rooming patients as needed.



Qualifications:

Education/ Experience:

  • Graduate of accredited school of nursing
  • Current LPN or RN licensure in Vermont
  • Prior experience in primary care preferred

Other Requirements:

  • Current Basic Life Support certification (AHA only)
  • Excellent critical thinking and judgement skills
  • Excellent customer service skills
  • Experience with Electronic Medical Records Systems

Requirements

Essential Functions Of The Position:

  • Triages patients’ needs over the telephone using approved protocol processes to determine primary presenting problem and severity of problem
  • Determines the appropriate plan of care and gives nursing advise over the telephone within scope of practice
  • Reviews incoming faxes for urgent medical conditions and notifies the provider immediately. Scans faxed records considered urgent into the Cerner system and tags the provider.
  • Schedules and/or makes referrals for appropriate care such as same day appointment within primary care or emergency department care
  • Communicates with GCFHH providers and nursing staff to ensure that appropriate care is implemented and delivered
  • Follow-up with patients, family members, and outside agencies as appropriate
  • Collaborates with scheduling and registration staff to ensure patients’ needs are meet
  • Perform all other related duties as assigned or requested by supervisor
  • Completes annual education certification and/or license requirements
  • Attends/completes mandatory in-services and trainings
