Palliative Care Advanced Practice Provider - APP (NP) - UNC Health Palliative Care at Lenoir

UNC Medical Group is seeking a 100% In-Patient, Full-time Palliative Nurse Practitioner to join their inpatient provider team on the campus of UNC Health Lenoir in Kinston, NC and Onslow Memorial Hospital in Jacksonville, NC. The Nurse Practitioner will be responsible for providing clinical care, support, and education for the palliative patient population as part of the health care team. The Nurse Practitioner will also provide excellent patient care services through inpatient consultation utilizing expert knowledge and competencies from a foundation of evidence-based practice in palliative care. This position will proactively identify opportunities to improve the patient and family experience related to acute, chronic, and/or progressive disease.

 Position Highlights:

  • Full-time: Monday-Friday: 8:00 am to 5:00 pm
  • Schedule: Weekly UNC Health Lenoir and Onslow Memorial Hospital rotation
  • Perform palliative care consultations and follow-up visits for hospitalized patients with acute palliative care needs including symptom management and complex medical decision making
  • Facilitate family meetings related to current medical decision making and advance care planning
  • Assist with developing appropriate discharge plans with respect to established goals of care for patients to ensure access and continuity of care
  • Actively participate as a member of an interdisciplinary team to ensure that palliative care is provided in a manner which addresses pain and symptom management; psychosocial/spiritual support; and caregiver support
  • Physician supervised, yet highly independent position that involves clinical practice and other support services to meet the palliative care needs of patients in the hospital
  • EMR: EPIC (Lenoir) and Meditech (Onslow)

 

Special Skills & Training:

  • Master’s degree in nursing.
  • Palliative, Internal Medicine or Geriatric APP work experience required
  • Experience appropriate to complex case management with issues of aging.
  • Nurse Practitioner licensed in the State of North Carolina & graduate of an accredited Nurse Practitioner program. Prescriptive authority in the State of North Carolina. Current BCLS certification.
  • National Hospice & Palliative Care certification desired.
