**Job Title: Caregiver 2PM-10PM**

**Job Description:**

We are seeking a compassionate and dedicated Caregiver to join our team at The Forum at Lincoln Heights and provide exceptional support to our clients. As a Care Manager, you will play a crucial role in enhancing the quality of life for individuals who need assistance with daily activities. You will join a team committed to maintaining the dignity, comfort, and independence of each client.

**Key Responsibilities:**

- Assist clients with personal care activities such as bathing, dressing, grooming, and toileting.
- Prepare and serve meals according to specific dietary needs and preferences.
- Perform light housekeeping tasks to maintain a clean and safe environment.
- Offer companionship and engage clients in meaningful conversations and activities to promote mental wellness.
- Assist clients in mobility and ambulation, ensuring their safety at all times.
- Monitor and report any changes in the client's health or behavior to healthcare professionals or family members.
- Administer medications according to the prescribed schedule.
- Accompany clients to appointments or errands as needed.
- Collaborate with healthcare providers and family members to create individualized care plans.

**Qualifications:**

- Previous caregiving experience or a background in a related field is preferred.
- Certification in First Aid and CPR is an asset.
- Excellent interpersonal and communication skills.
- Ability to handle sensitive and confidential information with discretion.
- Strong organizational skills and attention to detail.
- Empathy, patience, and a genuine desire to assist others.
- Ability to work independently and as part of a team.
- Flexibility to work various shifts, including nights and weekends, as required.
