Description Position Summary: Provides clinical knowledge and expertise in coordinating patient care in the Grace Cottage Family Health setting as directed by the medical provider. Qualifications: Education/ Experience: Certification per American Association of Medical Assistants preferred (AHA) , or LPN License. Other Requirements: Proficient computer skills required. Experience working with an electronic health record preferred. Requirements Essential Functions Of The Position: Treats patients and their families with respect and dignity. Facilitates patient flow throughout the encounter. Completes chart prep, driven by visit type and chief complaint including reviewing the medical chart for current information regarding the patient’s medications, test results, hospital records, operative reports, notes from consulting specialists, etc. Any missing results or other information should be requested and obtained. Maintains and updates clinical information including identifying and documenting active problems, completing/resolving historical problems, updating social and family history, reviewing and updating allergy list, reconciling patient’s current medication list, and determining if the patient needs refills after documenting any reported side effects of medications. Records chief complaint for visit, as obtained through patient interview. Performs patient intake including obtaining vital signs, contributing to the assessment of the patient’s health status, and recording results in the EMR. Maintains and updates preventative health screening documentation. Identifies psychosocial needs of patients utilizing the depression screening tool and communicates concerns to the patient’s medical provider. Utilizes nursing standards of care including assessment, plan, implementation and evaluation. Communicates the patient’s condition to the medical provider including any significant changes noted during patient assessment. Performs point-of-care testing per established policies and procedures. Assists medical provider with procedures. Maintains aseptic technique during treatments and special procedures. Licensed Practical Nurse (LPN) - Emergency Department, Mid Shift

Overview Our Emergency Department unit has 43 beds staffed by RN’s, LPN’s, Paramedics, EMT’s, and LNA’s. We are the only Level 1 Trauma Center in the state of NH and work closely with our system affiliates to accept patients requiring critical/neurological/cardiac/obstetrical/medical or trauma care. Our unit sees a large volume of referrals, transfers and trauma consults from outside facilities and provides services for the local community. We see 31,000 patients annually to include neonates through geriatric populations in various states of health, often times with multiple diseases processes. Emergency Nursing is a specialized area of practice that is both independent and collaborative, requiring the continual acquisition and application of a specialized body of knowledge and skills. This demands a broad scope of practice to promptly deliver emergency, urgent, and non-urgent care to patients of all ages and from all cultural backgrounds. Emergency nursing care is episodic, primary, and typically acute, but may be chronic in nature requiring knowledge and skills to care for patients of all ages, acuities, and physical or psychological conditions. ROTATING MID SHIFTS 9-9 or 11-11 and INCLUDEs ALTERNATE WEEKENDS Responsibilities The Licensed Practical Nurse – Inpatient/Procedural is responsible for providing safe, high quality care to general care patients as a team member in an assignment supervised by a Registered Nurse. Demonstrates commitment to patient/family engagement and effective communication. Practices consistently with applicable state nursing practice regulations, applicable D-H policies and procedures. The LPN-IP/Prcd may administer intravenous medications upon successful completion of NH Board of Nursing approved training and with specific delegation by a Registered Nurse. Provides and documents nursing care assessments, nursing interventions and patient/family education to stable patients under the supervision of a Registered Nurse in accordance with the patient’s plan of care. Alerts Registered Nurse to any observed change in patient presentation or unexpected assessment findings. Reviews assigned patient plan of care and pertinent active orders in the patient medical record prior to providing care. Assists assigned patients with activities of daily living, nutrition and hydration according to orders and goals of care. Administers oral, topical and inhaled medications. Applies dressings and assistive devices as indicated by assessment or as ordered by the interdisciplinary team, if applicable. Reports relevant assessment findings and suggestions to enhance patient care directly to members of the interdisciplinary team in addition to the supervising Registered Nurse.
