Position Summary:
Our dedicated Chronic Care nursing team make a lasting difference in the lives of patients with complex, long-term medical needs. Here, you will build meaningful relationships and provide compassionate, holistic care that promotes independence and quality of life. Key Responsibilities:
- Provide compassionate, evidence-based nursing care to patients in a long-term acute care or rehabilitation setting.
- Collaborate with physicians, therapists, and other care team members to develop and implement individualized care plans.
- Monitor, assess, and document patient progress while ensuring safety and comfort.
- Educate patients and families on treatment plans, medication management, and discharge planning.
- Support a culture of excellence, teamwork, and continuous learning.
Qualifications:
- Required: Current Registered Nurse (RN) license in the State of Connecticut.
- Required: Graduate of an accredited school of nursing (BSN preferred).
- Required: Demonstrated competence in the clinical setting.
