About the Role
We’re seeking a dedicated Registered Nurse (RN) to join our PACE interdisciplinary team on a PRN basis. In this flexible role, you’ll provide as-needed support to the clinic and interdisciplinary team—delivering high-quality, participant-centered care that promotes health, independence, and dignity.
How You’ll Make a Difference
Perform daily nursing tasks and prepare the clinic for participant visits.
Communicate effectively with staff and keep the Clinic Manager informed of participant needs or concerns.
Respond to participant emergencies and provide timely clinical assessments.
Participate in care planning, implementation, and evaluation of individualized participant care.
Document all nursing care accurately in the electronic medical record.
Triage participants efficiently and support the interdisciplinary care process.
Administer medications and maintain proper medication inventory, including contingency and emergency supplies.
Perform phlebotomy procedures and other clinical tasks as needed.
Ensure the cleanliness and organization of the clinic and medication areas.
Provide wound care and maintain detailed wound documentation.
Schedule and coordinate medical appointments and assessments with providers.
Participate as a key member of the interdisciplinary team to deliver holistic care.
Oversee medication passes and monitor for medication error trends and quality improvement.
Perform other related duties as assigned.
What You Bring
Current Registered Nurse (RN) licensure in the Commonwealth of Virginia.
Current CPR certification.
Minimum of one year of experience working with the frail or elderly population.
Medical clearance for communicable diseases and up-to-date immunizations prior to participant contact.
Valid driver’s license, proof of insurance, and reliable transportation.
