Position Summary:

The MDS Coordinator is responsible for the coordination of all aspects of the RAI Process. This individual will be required to coordinate with other departments on ARDs, Completion and submission timeline. Completion of Care Plans related to CAA Process. Provide training, education to staff as needed.

Qualifications of Registered Nurse Assessment Coordinator (RNAC):

  • High school diploma or equivalent.
  • Previous work experience in long term care MDS
  • Valid RN License
  • Valid Pennsylvania driver’s license.

JOB DESCRIPTION:

  • Proficiency in MDS 3.0
  • Knowledge of PDPM Process
  • Demonstrable knowledge of state and federal regulations
  • Monitor Medicare assessment schedules and nursing documentation to ensure accuracy and timely submission
  • Monitor Case Mix Index (CMI) scores, looking for potential risks and/or changes that may affect Medicaid reimbursement
  • Maintain current working knowledge of Medicare criteria, serving as a resource for nursing staff and communicate changes in regulations
  • Complete and assure the accuracy of the MDS process for all residents
  • Timely completion and submission of the MDS
  • Determine potential PDPM Reimbursement and expense associated with a potential admission
  • Oversee UR Process
  • Audit of skilled documentation daily
  • Ensure clinical is updated upon admission and weekly post UR on skilled documentation needs
  • Timely Manage Care Updates
  • Review of MDS Scrubber Report with IDT
  • Medicare Certs for Skilled Level of Care, including, initiation and updating timely
  • Monthly Triple Check- Review of UBs for billing accuracy
  • PA CMI- Reconciliation of CMI per picturedate, upload signed copy to NFRP site andretain original with MS Census.
  • Send preliminary and final CMI to RDCR
  • Review/Update diagnosis list in PCC with OBRA Schedule
