Position Summary:

Full time 40 hours per week, Monday - Friday, 8 hours per day

Full-time nurse practitioner, or physician assistant position available immediately to provide primary care services to frail participants of the Neighborhood PACE of NeighborHealth program, a nationally recognized service delivery model for community frail older adults. Neighborhood PACE is a founding member of the national PACE program (Program of All-Inclusive Care for the Elderly), now in operation for 35 years.

Care in all settings is provided in concert with an interdisciplinary care teams of health professionals in a progressive health care organization.

Candidate will provide geriatric primary care services for PACE participants across multiple long term care settings, including in the PACE clinics, at home , and in assisted living and nursing facilities.

Strong preference for a candidate with significant experience as a practitioner for frail adults. Spanish proficiency preferred but not required.

Candidates with Adult or Family Medicine certification will be considered.
