Job Summary

Under general direction and in collaboration with the multidisciplinary transplant team, the nurse coordinator provides expert nursing care for transplant patients, including candidates (pre-transplant), recipients (post-transplant) and/or living donors with a variety of complex treatment options. The nursing coordinator serves as a contact point for transplant patients, family members, referring physicians, staff as well as contact point for the coordination and facilitation of the care of the patient in specialty areas. The nurse coordinator provides leadership for collaborative and multidisciplinary patient care with emphasis on education and continuity of patient care for the entire period of care including inpatient, outpatient or both. The transplant nurse coordinator is responsible for monitoring and coordinating the care of transplant patients to ensure they reach their maximum health potential before and/or after transplant.

Specific daily responsibilities include: triaging patient calls and lab results, coordinating studies and specialty referrals, confirming orders, providing ongoing transplant patient education, spearheading the coordination of care for the transplant patient across disciplines, and clinic work as needed. Pre transplant coordinators will focus on the evaluation and workup of the pre transplant candidate, while the post-transplant coordinator will focus on coordinating the care and optimizing the health of the transplant recipient. Living Donor coordinators will focus on evaluation and workup of the potential living donor.

This position will support the transplant team by communicating with patients on behalf of providers. Respond to changes in patient condition as exhibited in signs, symptoms and lab values in collaboration with the provider. The nurse will make changes to medication and treatment plans as delegated by the provider. The nurse coordinator will educate patient on any changes and document in the EHR.

The final salary and offer components are subject to additional approvals based on UC policy.

Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this position classification at UCSF. For positions that are represented by a labor union, placement within the salary range will be guided by the rules in the collective bargaining agreement.

The salary range for this position is $95.76 - $124.25 (Hourly Rate).

Required Qualifications

  • Minimum two (2) years of outpatient employment in the field of organ transplantation or donation, Or equivalent experience in organ specific specialty (i.e. outpatient nephrology experience for kidney transplant, pulmonology experience for lung transplant, hepatology for liver transplant.)
  • Active Registered Nurse licensure in the State of California
  • Active American Heart Association CPR certification
  • Knowledge of end stage organ specific disease.
  • Understanding contraindications to transplant as well as common post-transplant complications.
  • Proven organizational skills with the ability to accept full responsibility for a defined and delegated area.
  • Excellent communication/interpersonal skills
  • Proven ability to work effectively with physicians and other support staff
  • The flexibility to orient and work at all UCSF Medical Center locations

Preferred Qualifications

  • Prior outpatient transplant experience
  • Solid Organ case management experience
  • Experience in immunosuppressive therapy

License/Certification

  • Active Registered Nurse licensure in the State of California
  • Active American Heart Association CPR Certification
