The Dialysis Home Therapies Instructor (RN) coordinates the administrative and clinical aspects of the home dialysis program, including home hemodialysis and peritoneal dialysis treatment options, and communicates with patients and support staff to ensure the highest standards of care.

 

Schedule: Fulltime, Monday through Friday; on call as needed

 

Compensation: Pay range from $38.50-$48.78 per hour, depending on nursing and dialysis experience

Responsibilities

What You Can Expect:

  • Collaborate with medical team on patient care concerns, hospitalizations, discharges and/or modality changes, etc.
  • Coordinate home training sessions, clinic visits, patient care conferences, home visits and all clinical and educational activities to assure efficient and productive use of nursing hours and program facilities
  • Monitor patient supply ordering and usage while maintaining quality care
  • Communicate regularly regarding home program activities with interdisciplinary care team
  • Monitor ongoing patient care parameters
  • Perform all registered nurse functions and duties
  • Create and maintain appropriate record keeping, patient training and documentation in accordance with ESRD Network, regulatory agencies, DCI CQI program and the dialysis facility's policies and procedures

Qualifications

Duties Description PRIMARY DUTIES AND RESPONSIBILITIES: Work collaboratively with the team of Article 16 Clinic nurses to provide the following services to OPWDD individuals under our care: Provide/prepare for routine and emergent individuals care including assessing/triaging individuals and obtaining vital signs, EKGs, performing finger-sticks, catheterizations, IM and subcutaneous injections (immunizations, Prolia, B-12, Haldol, etc), assisting with minor procedures and annual physicals, helping with Eye Clinic, etc. Process orders, consultation forms, prescriptions, etc. generated from above visits correctly and in a timely manner. This also includes creating/processing referrals to outside specialty providers. Create/process electronic prescriptions, durable medical equipment justifications and prior authorizations for medications and diagnostic procedures. Triage incoming phone calls, labs, mail, email and consults ensuring timely follow up by practitioner and compliance with Article 16 rules. Organize practitioner workload in an equitable manner throughout the day. Be liaison between Residential RNs/staff and Clinic practitioners (and other Article 16 practitioners as necessary) to ensure individuals needs are met in a timely manner. Create laboratory requisitions/prescriptions for routine and PRN lab work using correct ICD codes. Draw/process labs including blood, urine, stool, cytology and pathology specimens. Sterilize reusable equipment as appropriate and perform monthly testing and routine maintenance of sterilizer. Maintain up-to-date supplies for minor procedures, lab draws, immunizations, sterilizer/EKG/Glucometer, etc. Maintain Immunization and Lab fridge/freezer including monitoring temperature, cleaning and defrosting as needed.
