This is a Full Time (80 hours per pay period), 8-hour Day Shift Monday - Friday opportunity located at Scripps Memorial La Jolla.
Our Vascular - Nurse Navigator: coordinates patient care including being the initial contact for patients when referred to our team.
The Specialty Nurse Navigator will provide patient education regarding the referral, procedural, and post-procedural process. Additionally, will review patient’s records and help determine initial screening tests, and will be the point of contact for patient phone calls and will assure all screening appointments have been ordered. Will review screening tests' results and enter TAVR database. Collaborates with TVT registry abstractors to ensure data integrity. Will work with TAVR team to improve flow and processes. Works in collaboration with structural NPs and Cardiologists to review data for related projects.

As a Specialty Nurse Navigator, you will be responsible for:
- Formulating custom care plans from assessment and clinical information demonstrating the application of clinical guidelines including standards of care.
- Establishes specific, measurable, attainable, relevant and time specific nursing goals to guide the care planning process and promote self-reliance.
- Assists patients in establishing, modifying, or reinforcing a social support network, thereby promoting self-reliance.
- Identifies and aligns resources that impact high-risk care.
- Coordinates services on behalf of the high-risk patients.
- Work in collaboration with the Healthcare team to track and review data for research projects as well as resource utilization related to relevant specialty and make recommendations as needed.
- Vascular-Specific Patient Management
- Act as the point person for emergency inpatient cardiology consults, procedures and coordination of care.
- Support pre- and post-operative education, discharge planning, and follow-up logistics.
- Database Management
- Review data for accuracy, organize, and report quality data for internal review and ACS verification.
- ACS Accreditation and Quality Improvement
- Lead preparation for vascular ACS verification, including policy development, compliance tracking, and data abstraction.
- Partner with physicians on quality initiatives and assist with morbidity/mortality data reviews.
- Program Development
- Assist with formalizing protocols for limb salvage, aortic program growth, and evidence-based care pathways.
- Help establish standardized order sets and care algorithms.
- Liaison Role
- Serve as point of contact between vascular surgery, cardiology, heart failure, and ECMO/MCS teams where relevant.
- Ensure smooth communication and coordination for multidisciplinary patient care.
- Administrative and Regulatory Support
- Ensure compliance with hospital policies, JCAHO standards, and documentation best practices.
- Track patient outcomes and help with case reviews and institutional reporting
Required Qualifications
- Current California RN License
- Current American Heart Association BLS and ACLS
- Three (3) years of recent RN experience in a clinical setting
- Knowledge of vascular patient care
- Bachelor's degree in nursing (BSN)
- Master's degree in nursing
- Two years of experience managing care of relevant specialized patient population.
- Five (5) years of recent RN experience in a Clinical Setting
- Experience working in a Interventional Radiology, Cath Lab, Cardiac Step Down, or CVICU
