Core Responsibilities
As a Licensed Practical Nurse (LPN) at our facility, your crucial contributions and clinical leadership will include:
Medication Administration: Accurately administering medications to residents in strict accordance with their individualized care plans and physician orders, adhering to the "five rights" of medication safety.
Treatment & Procedures: Skillfully performing various treatments and procedures assigned by the physician's orders, always operating strictly within your professional scope of practice.
Observation & Reporting: Thoroughly reporting and meticulously recording the resident's condition and the care provided, including any subtle or significant signs and symptoms that may indicate a change in status to the Charge Nurse or RN promptly.
Care Coordination: Collaborating with the Registered Nurse (RN) and interdisciplinary team to implement, evaluate, and update resident care plans, ensuring holistic and effective care delivery.
Compliance: Ensuring all nursing duties are performed in strict adherence to local, state, and federal guidelines, maintaining the highest standards of care and regulatory compliance.
Documentation: Maintaining accurate and detailed clinical documentation, including transcribing physician orders accurately, to ensure continuity of care.
Undertaking other duties as assigned by the Unit Manager, Director of Nursing, and Administrator, contributing to the smooth and effective operation of the facility.
Qualifications
We're looking for individuals who demonstrate:
License: Must possess an active and unrestricted Licensed Practical Nurse (LPN) license in the state of Wisconsin.
Teamwork: The ability to work effectively both independently and as a collaborative member of a high-functioning healthcare team.
Tech Savvy: Proficiency in basic computer skills for electronic health record (EHR) documentation and communication.
Experience: Previous long-term care or skilled nursing facility experience is preferred, though not strictly required; we are open to training motivated individuals and new graduates.
