Responsibilities:

  • Conduct thorough patient assessments, diagnose conditions, and develop comprehensive care plans.
  • Manage and treat complex medical conditions with a focus on reducing hospital readmissions.
  • Document patient encounters in electronic health records (EHR).
  • Collaborate with physicians, nursing staff, and other healthcare professionals to ensure seamless patient care.
  • Communicate regularly with patients' families and support systems.
  • Attend facility meetings and participate in care planning discussions.
  • Maintain professionalism and uphold Complete Care’s commitment to excellence in patient care.

Qualifications:

  • Master’s or Doctorate degree in Nursing from an accredited program.
  • Active Nurse Practitioner (APN) License and Registered Nurse (RN) License in New Jersey.
  • DEA, CDS, and BLS certifications.
  • Minimum 2 years of Nurse Practitioner experience (post-acute care experience preferred).
  • Strong clinical skills, including IV insertion, G-tube replacements, wound care, and suture/staple removal (preferred).
  • Experience managing elderly patients and complex medical conditions.
  • Proficiency in electronic health records (EHR).
