Primary Job Responsibilities

Provide expert nursing to heart transplant patients
Systematically assess the need for and the delivery of nursing services that meet patient care requirements, whether delivered directly or by the other nursing staff
Ability to formulate, coordinate and revise an individualized plan of care as indicated by the patient’s response to treatment and evaluate overall plan daily for effectiveness
Provide care for individual patients over the telephone using the nursing process
Perform all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/healthcare errors
Integrate patient and family education into the delivery of care, ensuring educational activities are culturally appropriate and are tailored to the patient’s and family’s ability to comprehend and participate in the post-heart care
Coordinates the care given by others to achieve efficient clinic operations
Assume responsibility for continued professional growth and sharing expertise with other team members

Uses health assessment data to determine nursing diagnosis. 6. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. 7. Initiates appropriate preventive and rehabilitative nursing procedures. Graduate of an accredited school of nursing 2. Registered Nurse, with a License in good standing to practice in the state of Washington. 3.
