Licensed Practical Nurse Case Manager - Employee Health; $18,750 3 years Sign On, $15,000 Relocation, and Retention Bonus Eligible

Position Summary: This position is accountable for assessing, planning, coordinating, monitoring and evaluating services for patients and their families, thus fostering quality, continuity and appropriate utilization of health care resources throughout the continuum of care.  LPN Case Managers facilitate a cooperative effort between patients, parents, doctors, nurses, payers, and allied health care workers, community agencies and home care services, thus promoting a smooth journey through the entire span of care.  The case manager facilitates the coordinated utilization of resources for maximization of health outcomes, patient/family satisfaction and financial outcomes.

Position Qualifications:

Minimum Education: Graduate from an LNL school for LPN's or LVN's.  In addition, must be licensed as an LPN in the State of Alaska in good standing. 

Minimum Experience: 3 - 5 years prior experience in nursing.

License, Certification, Registration: Current CPR certification (BCLS) required.  Must be licensed in good standing in the State of Alaska as a Registered Professional Nurse.  Current BLS.  The successful candidate will be required to pass and maintain competencies for this position.

Equipment/Tools: Office machines (i.e. computer, copier, fax, telephone, Microsoft Office)

Specialized Knowledge and Skills:

1.         Knowledge of medical terminology.

2.         Ability to work in patient care setting.

MAT Clinical Nurse Care Manager - LPN

Position Summary: This position is accountable for assessing, planning, coordinating, monitoring and evaluating services for patients and their families, thus fostering quality, continuity and appropriate utilization of health care resources throughout the continuum of care. The Clinical Nurse Care Manager will help coordinate the various aspects of the Medication Assisted Therapy (MAT) program and act as a bridge between the Behavioral Health and Medical components of the treatment program. The role combines direct patient care in combination with all required details of the care management of patients who utilize the MAT program for the treatment of substance use disorder. The Clinical Nurse Care Manager will collaborate with MAT medical providers, MAT pharmacists, MAT behavioral health clinicians and counselors, and the staff of YKHC scheduling, YKHC travel, and village services to coordinate the timely and effective care of both current and prospective patients utilizing the MAT program to promote the best outcomes. Specific responsibilities and skills include but are not limited to: • provides skilled nursing services to opioid dependent patients and their significant support systems • possesses sound clinical judgment in decision making • engages in patient care assessment, understanding patient care goals as voiced by the patient, making the patient a major part of his or her treatment and recovery • provides advocacy as needed to ensure patient knows and understands his or her rights • helps the patient become independent by meeting care goals and measuring those goals against patient healthcare outcomes • documents and communicates nursing care to others on the care team • provides education for patients and/or families • meets documentation requirements in the patient medical record • acts as liaison between patients, providers and other team members • performs ongoing assessments, intakes, evaluations, triage, teaching, inductions, stabilizations, and monitoring of patients in the MAT treatment program • assists in the primary care of patients within the MAT treatment program • facilitates support and services provided at other settings • collaborates with team members within the organization and outside sites Position Qualifications: Minimum Education: Must have completed a program of education in nursing from an NLN approved institution.
