  • The Registered Nurse Care Manager supports the growth of our patient population, as well as manages complex patient cases involving chronically ill patients by implementing treatment plans and facilitating patient’s access to appropriate health care services and community resources. They educate patients, identify and remedy gaps in patient therapies, and manage therapy for patients enrolled in Care Management, which includes assessments, planning, and evaluating the patient’s response to treatment.

Primary Job Responsibilities

  • The RN2CM works as a Care Manager with patients by offering exceptional communication and an ability to expertly assess patient needs.
  • Works closely with Providers and applies professional skills and knowledge of established concepts, principles, and practice to provide care coordination to patients.
  • Assesses psychosocial needs and implements strategies to ensure patients receive the appropriate levels of care.
  • Completes a nursing assessment to include patient’s medical history, disease process, medications, laboratory data and manages therapy per provider’s treatment plan.
  • Communicates plan of care in a timely manner to provider, patient, and family to establish mutual goals.
  • Assesses patients/families’ knowledge, learning needs, readiness, and comprehension abilities.
  • Educates and coaches patients/families on disease states, behavior modification, and ED and resource utilization.
  • Applies nursing best practices to identify and prioritize healthcare triage interventions that are appropriate, effective and that reduce the risk of adverse outcomes.
  • Answers calls in a timely manner, follows policy to document call inquiries, maintains confidentiality of all caller and personnel issues, and transcribes patient information electronically and through telemedicine digital technology.

About Our Endoscopy Unit This position will work at our brand-new outpatient endoscopy suite in North Seattle Our outpatient suite has 4 procedure rooms and 12 admit/recovery bays RNs provide procedural support for diagnostic and screening GI services, including colonoscopies WORK SCHEDULE 4x10 shift schedule Day shifts, Monday - Friday The exact days worked may vary each week POSITION HIGHLIGHTS Join a state-of-the-art endoscopy suite Supervise a team that provides life-changing care for patients Work closely with the Nurse Manager to facilitate daily operations and projects Primary Job Responsibilities Acts as a lead point of contact for Registered Nurses and Medical Assistants Establishes effective workflows for routine tasks and projects Provides procedural nursing support, which includes providing patient care before, during, and after conscious sedation Facilitates staff meetings in conjunction with the Nurse Manager Reviews unit staffing schedule and plans for coverage due to vacations, LOAs, and vacant positions Monitors unit acuity and patient flow, and supports care coordination for complex patients Provides feedback to the Nurse Manager for annual performance evaluations Plans, implements, and evaluates unit orientation for staff Models service excellence in daily practice Demonstrates ability to provide “just in time” feedback to staff regarding clinical performance, interpersonal team interactions, and patient/family communication Promotes practices and decisions that reduce human error and promote staff/patient safety Requirements 2 years of recent endoscopic nursing experience Previous conscious sedation experience Previous charge, lead, or supervisory work experience BLS Active licensure to practice as a Registered Nurse in Washington State About Uw Medical Center-northwest UW Medical Center is an acute care hospital located in Seattle with two campuses: Montlake and Northwest.
