Minimum Qualifications:

  • Experience: A minimum of 1,000 hours (approximately 6 months full-time or 12 months part-time) of experience working with geriatric or chronically ill individuals is preferred. Exceptions made for recently licensed CNAs, HHAs, or those providing companionship and housekeeping only.
  • Certifications: CNA or HHA certification is preferred but not required.
  • Health and Safety: Proof of current TB testing, ability to safely transfer clients, and compliance with infection control protocols.
  • Transportation: Reliable transportation, valid driver’s license, and automobile insurance required for client transport.
  • Background Check: Must be able to pass a criminal background check (national and state-required).
  • Communication: Excellent communication skills for interacting with clients, family members, and team members.

Essential Duties and Responsibilities:

  • Care Plan Implementation: Follow personalized care plans to ensure clients receive the appropriate support and assistance.
  • Client Support: Provide companionship, meal preparation, housekeeping, transportation, and personal care (bathing, dressing, toileting, feeding).
  • Communication and Advocacy: Report any changes in client condition to the Care Manager and advocate for client needs.
  • Safety and Compliance: Adhere to safety protocols and infection control practices; notify the office if additional supplies are needed.
  • Timekeeping and Documentation: Accurately log hours, tasks, and client status using ClearCare Telephony system.
  • Professionalism: Maintain a professional appearance and conduct, adhering to company policies and dress code.

Physical Demands:

  • Ability to assist with client transfers and repositioning using proper techniques and equipment.
  • Reliable transportation to clients’ residences.

This may include the provision of hands-on personal care services as well as tasks generally considered to be companion or homemaker in nature, such as light housekeeping, meal preparation, shopping, errands, transportation and companionship. MINIMUM QUALIFICATIONS: 1,000 hours of long-term care with geriatric or chronically ill persons. This translates to approximately 6 months of full-time work or 12 months of part time work. Exceptions to this requirement are for recently licensed Certified Nursing Assistants (CNAs) or Home Health Aides (HHAs), or persons who will only be providing companionship and housekeeping services. CNA or HHA license preferred. Proof of current TB testing. Must be physically able to perform the duties of this position. Must have available reliable transportation; if transporting clients, must also have proof of a valid driver’s license and automobile insurance. Must possess excellent communication skills. Be mature, caring, punctual, and reliable. Pass a criminal background check (national and any required by state). Provide verifiable professional and personal references. ESSENTIAL DUTIES AND RESPONSIBILITIES: Understand a client’s care plan and adequately implement it. Assist clients according to their care plan which may include: companionship, meal preparation and service, light housekeeping, transportation, shopping, medication reminders, bathing, body care, dressing, feeding, ambulation, positioning, transferring, toileting, and protective supervision. Notify the Care Manager of client concerns, changes in care or client condition and client emergencies. Notify the Care Manager and/or Staffing Coordinator of any safety concerns. Follow company Infection Control practices; notify the office if in need of appropriate Personal Protective Equipment (PPE) or supplies. Follow the ClearCare Telephony Instructions to clock in and clock out for all scheduled shifts and accurately update your task status for each client prior to clocking out and leaving the client’s home. For compensable worked hours that are not allocated to a specific client (such as training time), you must use and submit the Time Log and Time Certification for Non-Visit Time form for each period pay period. Conform to the company dress code as specified in the Employee Handbook. Report to work on-time and ready for the day. Complete ongoing training as required by client care needs and/or changes in company policies and procedures. Attend staff meetings. Adhere to all company policies and procedures as detailed in the Employee Handbook. ORGANIZATIONAL RELATIONSHIP: The caregiver is supervised by the Care Manager and receives scheduling direction from the Staffing Coordinator. PHYSICAL AND ENVIROMENTAL DEMANDS: Reposition and transfer clients, sustain weight of clients using proper transfer safety techniques and equipment (example: use of transfer belt while assisting client to transfer from bed to chair), and travel to client’s residence place of residence using a reliable means of transportation.
