In this vital role, you will be responsible for enhancing the quality of life for our clients by providing essential assessments, developing care plans, and ensuring excellent communication among caregivers and clients. What You’ll Do

Be the First Point of Contact: Provide excellent customer service to clients (current and potential), industry partners, and care providers.
Schedule Assessments: Perform in-home assessments and re-assessments for clients.
Write Care Plans: Ensure Caregiver Managers have the necessary information to review with caregivers before care begins.
Assess Client Needs: Admit new or current clients for nurse delegation, medication, or other specialty care in accordance with state law.
Conduct Visits: Complete caregiver observation and client supervisory visits, documenting in client and caregiver files.
Update Care Plans: Communicate any changes to caregivers, route updates to clients for review and signature, and document accordingly.
Provide Training: Perform caregiver training and delegate additional training as needed.

Work Schedule

Hours: Expected to work 10-20 hours per week.
Typical Office Hours: Monday – Friday, 8 AM – 5 PM. Occasional evenings and weekends may be required.
Location: 60% of your time will be in an office environment; 40% will be in various client homes for assessments or observations.
