JOB DESCRIPTION SUMMARY:

The Certified Nurse Aide is a member of the Nurse Next Door team and works under the supervision of the Care Designer or a nurse and performs personal care services contained in a care plan written by a licensed health professional, employed or contracted by the agency. The Home Care Aide/Certified Nursing Assistant is responsible for observing clients, reporting these observations and documenting observations and care performed.

The Certified Nurse Aide will be assigned in a manner that promotes quality, continuity and safety of a client’s care.

JOB FUNCTIONS/RESPONSIBILITIES:

Responsibilities of the Certified Nursing Assistant include, are to, but are not limited to the following

As directed by the Care Designer or Nurse: client care  

As directed by the Care Designer or Nurse: Personal care

       a. Bathing 

  1. Grooming, including oral care

  2. Assist the client with dressing and undressing

  3. Assist the client with toileting activities, including the use of the bedpan and urinal

  4. Provide catheter care including changing the urinary catheter bag

  5. Empty ostomy bags, or changing bags that do not adhere to the skin

  6. Assist in feeding the client, if necessary

  7. Feed clients who are unable to feed themselves

  8. Take and record vital signs (temperature, pulse, respirations and blood pressure)

  9. Measure and record height and weight, if necessary.

  10. Support good skin care to prevent skin breakdown (i.e. turn and reposition client, actively

    encourage movement)

  11. Assist client with active and passive range of motions as defined by the plan of care

  12. Provide client with help walking and transferring; assist with the use of devices for aid to daily

    living such as a wheelchair or walker;

  13. Observe client’s physical, mental and emotional conditions

i. Report any change to the Care Designer

ii. Document observed changes

  1. Measure and document intake and output, if necessary

  2. Assist client in the self-administration of medication

  3. Verbally remind clients to take their medications

  4. Perform light housekeeping; maintains a clean, safe and healthy environment, which may

    include light cleaning and straightening of the bathroom, straightening the sleeping and living areas, washing the client's or client's dishes or laundry, and such tasks to maintain cleanliness and safety for the client;

  5. Perform meal preparation and clean up

  6. Change bed linens and make up the client’s bed

  7. Maintain an orderly client environment.

  1. Follow principles of infection control and Universal Precautions.

  2. Identifies and participates in performance improvement activities.

  3. Provide transport to doctor appointments, outside activities and shopping.

  4. Participates in client teaching according to the client’s plan of care.

  5. Assist in providing a safe environment and promote quality client care through adherence to

    established policies, procedures and standards.

  6. Ensure client’s rights are adhered to. These include but are not limited to:

    1. the right to privacy and dignity while care is being provided

    2. the right to accept or refuse care

    3. the right to confidentiality of their information

  7. Respond rapidly and appropriately in an emergency situation

  8. Use equipment properly and in a safe manner.

  9. Perform other tasks as determined and instructed by the licensed professional and as permissible

    under Idaho law.

    QUALIFICATIONS

  • Certified Nurse Aide current license 

  • High school diploma or GED

  •  Registered on the Idaho State Health Care Aide Registry

  •  Minimum of one (1) year of documented experience preferable

  •  Certified in CPR

  •  Criminal background check clearance

  •  Negative TB skin test or chest x-ray

  •  Licensed driver with automobile that is insured in accordance with the organizations requirements.

  •  Empathy for the needs of the client

  •  Demonstrate effective oral and written communication skills

  •  Ability to express spoken and/or written ideas in English

  •  Treats clients, staff and the public with courtesy, respect and presents appositive public image.

  •  Works as a team member

Certified Nursing Assistant Job Description

Ensures confidentiality and security of the client’s medical information.

JOB LIMITATIONS:

The Certified Nursing Assistant will not function in any manner viewed as the practice of nursing according to the State’s Nurse Practice Act. Specifically, the Certified Nursing Assistant will not administer medications, obtain physician’s orders or perform procedures requiring the training, knowledge and skill of a licensed nurse.

The CNA SHALL NOT:

  •  Change sterile dressings

  •  Irrigate body cavities such as giving an enema

  •  Irrigate a colostomy or wound

  •  Perform a gastric irrigation or enteral feeding

  •  Catheterize a client

  •  Administer medication

  •  Apply heat by any method

  •  Care for a tracheotomy tube

  •  Provide any personal health service which has not been included in the written service provision plan

    or care plan.

    ENVIRONMENT, PERSONAL PROTECTIVE EQUIPMENT OR PHYSICAL DEMANDS

Personal Protective Equipment: Physical Requirements:

Contact with clients under a wide variety of circumstances. Care provided in client’s living environment with varying situations or in an institution such as a hospital. May be required to respond in an emergency situation. Travel required. Position includes tasks that have the potential for exposure to blood and other potentially infectious material (OPIM) such as body fluids/tissues.

May include gloves, mask, eye protection, and disposable outer covering.

Must be able to stand walk, squat, bend, kneel, reach, twist, push and pull. Must be able to lift up to 35 pounds.
