Responsibilities:
  • Conduct thorough assessments of patients and their families to determine home care needs.
  • Provide comprehensive physical assessments and document the history of illnesses.
  • Develop and update individualized care plans with active participation from patients and their families.
  • Identify discharge planning needs and ensure they are addressed before patient discharge.
  • Offer counseling to patients and families to address nursing and related health needs.


Qualifications:
  • Current and valid state RN license. 
  • Minimum of 1 year of recent acute care experience in an institutional setting. 
  • Basic Life Support (BLS), with hands-on CPR certification. 
  • Valid driver's license and auto insurance. 
  • Associate's or Bachelor’s degree in Nursing, and 1 year of home health experience preferred.
