What You’ll Do:

Deliver direct patient care and treatments based on individualized care plans


Administer medications and tailored interventions


Promote wellness through education, counseling, and preventive care


Collaborate with multidisciplinary teams to coordinate care


Maintain accurate and timely nursing documentation


Assist with mobility and physical support (must be able to lift up to 50 lbs)


Stay active and engaged during extended periods of standing or movement


Take on additional duties as needed to support patient well-being

What You’ll Need:

Active LPN or RN license issued by New York State


Minimum 1 year of experience in a home care setting


Current CPR/BLS certification


Strong communication and organizational skills


Empathy, reliability, and a commitment to high-quality care
