Responsibilities 
  • Provide compassionate, personalized in-home care
  • Monitor client health to reduce hospital re-admissions
  • Create a safe, nurturing home environment
  • Encourage social interaction and enrichment activities

  • Accurately complete care tasks and documentation
  • Follow individualized care plans with professionalism
Qualifications & Skills
  • Must pass a South Carolina SLED background check
  • Minimum of 6 months experience with seniors or chronically ill individuals
  • CNA/HHA/PCA certification preferred
  • Excellent communication and documentation skills
  • Reliable, compassionate, and punctual
  • Valid driver’s license, auto insurance, and reliable transportation (if transporting clients)
  • Experience with WellSky is a plus
  • Must pass a 10-panel drug screen

Applicants must be a minimum of 18 years old, or the age required by state regulations, whichever is higher. Working experience alone, on a team, and on projects start to finish Strong emotional intelligence and listening skills Dedication to the medical field, especially towards working with the residents Must pass State and company background checks and drug test.
