Job Responsibilities

Working in contract role in a local acute facility for a designated time period (usually 10-15 weeks)
Providing quality patient care within your scope of practice for patients requiring a wide range of clinical needs
Assisting patients with pre-op and post-op procedures in coordination with the broader care team
Starting IV lines, inserting and maintaining catheters, dressing and managing wounds, and assisting with respiratory management
Taking and monitoring patient vital signs and conducting head-to-toe assessments
Ensuring medication administration and titration as prescribed
Identifying infection risks and implementing infection control best practices
Documenting patient conditions and nursing interventions and providing regular and accurate reporting to the wider clinical team
Conducting patient education regarding medical procedures, discharge planning, and continuity of care
Overseeing, and coordinating with, nursing assistants and other support staff
Adhering to facility policies and procedures and all relevant nursing standards of care
Being accountable to follow and adhere to patient care plans

Qualifications and Skills

Current Illinois state Registered Nurse (RN) license
Minimum of 8 months experience working as a Med Surg RN
Graduation from a nursing program at an approved institution (BSN preferred)
Successful completion of BLS and ACLS courses from an approved organization
Immunizations as determined by facility requirements
Completion of a standard background check
Ability to lift a minimum of 50 lbs

The RN Patient Educator, Nephrology collaborates with medical practitioners and leaders to incorporate chronic disease education into the plan of care for a specialized group of patients. The incumbent in this role is responsible for helping to improve patient health outcomes by promoting and facilitating disease education with the patient and/or family member(s) thereby ensuring their active involvement in care and care decisions. He/She performs direct specialty nursing services to patients as required, ensuring adherence to regulatory standards and all related policies, procedures, and guidelines. The RN Patient Educator, Nephrology, is also accountable for the development and delivery of clinical education to patients that increases their knowledge of disease processes, improves patient engagement in care, and reduces hospital and ER admissions. ESSENTIAL JOB DUTIES/RESPONSIBILITIES: Provides chronic disease education relative to quality care for a specific patient population.
