Eddy SeniorCare has a rewarding opportunity for a Licensed Practical Nurse (LPN) to work out of our 385 Watervliet Shaker Road, Latham NY location!

This is a full time position supporting both our primary clinic and our home based participants PACE is a unique program on the forefront of trends in healthcare, combining an insurance role (we are the Medicare/Medicaid insurer for our participants) with a comprehensive healthcare provider role (primary care, home health, adult daycare, transportation, all coordinated by an interdisciplinary team).

The primary purpose of this job position is to provide skilled nursing care and prescribed treatments to participants according to professional nursing standards and primary care orders. Care is performed at our center as well as in our participant's homes.

Duties Include:

Preparing participant visits in the medical clinic
Drawing labs, administering vaccines & collecting specimens
In addition, this role supports the management of our participants in the community including medication delivery and dispenser set up, wound care and other treatments as ordered.
The LPN may assist in the oversight of unlicensed staff, ie. home health aides.

Requirements:

Valid NYS LPN License
The candidate should have experience or willingness to become proficient in phlebotomy, and IV administration of fluids and medications within scope of practice.

Pay Range:$25.00 - $30.10
