Evergreen Commons is looking to hire a full-time, Monday-Friday day position, dynamic MDS nurse to work at our skilled nursing facility. This role includes flexible hours, mostly remote with 1-2 days a week on site. Your duties will include facilitating the MDS process and assessments of residents as necessary. You will also work with the medical and interdisciplinary team to develop patient care plans. MDS Nurse Responsibilities: Assessing and monitoring patients' health in accordance with medical laws, regulations, and guidelines. Collecting patient data and organizing the information into reports for further research. Communicating with health care teams to create applicable health care plans for patients. Assisting and alerting patients with any notice of non-medical coverage updates MDS Nurse Requirements: NYS RN license. Solid knowledge of MDS processes such as the Resident Assessment Instruments (RAI). Excellent organizational and multitasking skills. The ability to make independent judgments utilizing initiative, discretion, and the nursing process, as needed.
Weekend Warrior LPN Job duties: Plans and manages patient care according to each patient’s needs. Interviews patients and records their medical history and physical condition. Obtains patient vital signs, including pulse, blood pressure, temperature, and respiration. Provides routine care for patients. Observes patients’ health. Reviews medical charts. Accompanies patients to their rooms. Assists physicians and registered nurses. Communicates with patients and their families. Performs standard diagnostic procedures including EKG, phlebotomy, venipuncture, PFT, and Doppler. Prepares specimens for lab courier. Administers medications. Provides wound care. Turning and repositioning bedridden patients. Ensuring patients or residents receive appropriate diet by reviewing their dietary restrictions, food allergies, and preferences. Obtaining a wide range of information from physicians, caregivers, and nurses about patient conditions, treatment plans, and suggested activities. Measuring and recording food and liquid intake and urinary and fecal output and reporting changes to medical or nursing staff. Recording vital signs, including blood pressure, pulse, temperature, and respiration rate as requested by staff. Examining patients to detect issues requiring medical care, including open wounds, bruises, or blood in the urine.
