Evergreen Commons is looking for a part time Transportation Aide to attend resident medical appointments within the community. This individual must have an active CNA certification that is willing and able to provide direct patient care. This individual must posess a high level of customer service and is punctual with attendance. Duties when not on appointments and at the facility: • Makes rounds on the unit(s) looking for items needing attention; • Straighten utility rooms, linen rooms and pantry as needed; • Assist with meal service. • Deliver mail to residents; opening and reading mail to the resident as requested by the resident; • Make beds and tidy rooms; • Pass clean linen; • Assure ice water is available on the units; • Prepares rooms for new admissions; • Makes inventory of resident’s personal possessions and sends items to personal laundry to be labeled. • Assist with ADL care for residents • Assist with doctor visits at the facility Why join our team? Evergreen Commons Rehabilitation & Nursing Center is located just outside of Albany in East Greenbush, NY and we are looking for an experienced Assistant Director of Nursing. The chosen candidate will be energetic and demonstrate great leadership qualities. At Evergreen Commons we have 2 ADONs that work closely with the DON. This helps with coverage, if necessary and increases overall efficiency within the department. 1 ADON who is already on staff handles all things infection control (including COVID), tracking of hospitalizations, oversees 3 unit managers and in charge of supplemental auditing. The other ADON handles wounds, medication regimes (including pharmacy recs and GDRs), oversees the remaining 3 unit managers and completes other supplemental auditing. You will assist in overseeing the nursing team ensuring quality customer service and compliance with all Federal, State and local rules and regulations, as well as our policies and procedures. The ADON will be in charge of specific dutes within our nursing department. • The ideal candidate will have prior experience working in a Long Term Care setting. • The ideal candidate will have completed the 19.3 hours CDC Nursing Home Infection Preventionist Training Course. This is not required however preferred. • Coordinates and directs the total planning for nursing services to meet the needs of all residents. REQUIREMENTS: • Current New York RN unencumbered license required • Min. 3 yrs. exp. in Long-Term Care settings • 2 years management experience preferred • 1 year staff education experience preferred • Excellent Communication Skills required • Evidence of great leadership skills • Comfortable with computer systems and electronic documentation This is a full time, exempt position. Evergreen Commons is looking to hire a full-time, Monday-Friday day position, dynamic MDS nurse to work at our skilled nursing facility. This role includes flexible hours, mostly remote with 1-2 days a week on site. Your duties will include facilitating the MDS process and assessments of residents as necessary. You will also work with the medical and interdisciplinary team to develop patient care plans. MDS Nurse Responsibilities: Assessing and monitoring patients' health in accordance with medical laws, regulations, and guidelines. Collecting patient data and organizing the information into reports for further research. Communicating with health care teams to create applicable health care plans for patients. Assisting and alerting patients with any notice of non-medical coverage updates MDS Nurse Requirements: NYS RN license. Solid knowledge of MDS processes such as the Resident Assessment Instruments (RAI). Excellent organizational and multitasking skills. The ability to make independent judgments utilizing initiative, discretion, and the nursing process, as needed.
Weekend Warrior LPN Job duties: Plans and manages patient care according to each patient’s needs. Interviews patients and records their medical history and physical condition. Obtains patient vital signs, including pulse, blood pressure, temperature, and respiration. Provides routine care for patients. Observes patients’ health. Reviews medical charts.
