A bit about this role:

This is an exciting opportunity for a compassionate and skilled Nurse Practitioner (APRN) to provide virtual primary care through our innovative telehealth platform. As a Virtual Primary Care Nurse Practitioner, you will deliver comprehensive, high-quality care while fostering strong, lasting patient relationships.

You will provide ongoing management of chronic conditions, preventive screenings, acute care visits, and medication management, all through a virtual care model. You'll also work collaboratively with a multidisciplinary team to coordinate in-person services such as lab work, imaging, and specialist referrals as needed.

Your Responsibilities and Impact will include:

  • Provide virtual primary care visits via telehealth, including comprehensive assessments, chronic disease management, preventive screenings, and acute care interventions.

  • Diagnose and manage chronic conditions (e.g., hypertension, diabetes, COPD, heart disease) and develop individualized care plans to optimize patient health.

  • Evaluate and treat acute concerns (e.g., infections, respiratory illnesses, minor injuries, skin conditions) within the scope of virtual care.

  • Order and interpret lab tests, imaging, and other diagnostics, coordinating follow-up care as necessary.

  • Manage medication prescriptions, titrations, and refills, ensuring patient adherence and safety.

  • Provide health coaching, lifestyle modification guidance, and patient education to empower patients in managing their health.

  • Identify patients needing referrals, or home-based services, and facilitate appropriate coordination with specialists or community resources.

  • Utilize and provide feedback on our electronic health platform, contributing to its continuous improvement to enhance patient and provider experience.

  • Participate in clinical case reviews, team huddles, and interdisciplinary collaborations to ensure high-quality, patient-centered care.

  • Engage in continuous learning and improvement, staying up-to-date with best practices in virtual care and primary care management.

Required skills and experience:

  • Active Nurse Practitioner (APRN) license in good standing in [Required States] or the ability to obtain licensure in multiple states within 90 days of hire.

  • Board certification in Family Medicine, Adult-Gerontology, or Internal Medicine.

  • Minimum 2 years of primary care, urgent care, or internal medicine experience, preferably with experience in telehealth or virtual care settings.

  • Active BLS certification at time of hire.

  • Shift times may vary in order to align with standard business hours (9:00 AM – 5:00 PM) in the patient’s local time zone. Flexibility is required. Additionally, weekend shifts may be required in the future based on evolving patient care needs.

Desired skills and experience:

  • Experience managing chronic conditions, preventive care, and acute illnesses via telehealth.

  • Familiarity with value-based care models, population health management, and quality metrics (HEDIS, STAR ratings, etc.) is a plus.

  • Comfortable with digital documentation and telemedicine platforms (experience with EHRs and virtual care workflows preferred).

Salary range: $115,000 - $150,000 annually

Physical: The incumbent must be able to walk several hundred yards quickly, write well, hear well (for cardiac examinations) and see well for complete diagnostic skills. Work requires prolong periods of regular and recurring standing and walking. This position requires the frequent ability to sit, drive, bend, climb, kneel, crouch, twist, maintain balance and reach. The incumbent will know how to use appropriate body mechanics and proper lifting techniques as needed to assist in set up and take down of equipment and clinic from the mobile medical unit daily. The incumbent must be able to lift, push and pull up to 24 lbs. prolonged and up to 50 lbs. frequently and occasionally over 100 lbs. Work requires extensive driving between communities, ability to sit for long periods of time on RV Mobile Medical unit. Must have ability of prolonged seeing fine details, hearing normal speech and telephone use. The Incumbent must be ability to frequently use near vision, far vision, color vision, depth perception, and hear overhead pages on a loudspeaker. Must have ability of bilateral hand manipulation-frequent use of simple and firm grasping, fine manipulation; and, prolonged use of a keyboard. Mental: The interaction with emotionally disturbed patients is common. Also, must be able to work evenings on a regular basis and handle the stresses of rural medicine. Nurse Practitioner must be able to cope and manage stressful situations such as chronic or terminal illnesses, child or elderly abuse, and social issues or concerns among patients and families. Must be able to continuously concentrate, work alone, work in areas that are close and crowded, frequently cope with high levels of stress, make decisions under high pressure, manage altercation, handle multiple priorities in stressful situation, demonstrate high degree of patience, adapt to shift work, and occasionally cope with anger/fear/hostility of others in a calm way and handle high degree of flexibility. Must accept a flexible schedule to meet unit needs. Environmental: Work is performed in a clinic where exposure to infectious disease is common. May also frequently be exposed to chemical agents, dust, fumes, gases, extremes in temperature or humidity, hazardous or moving equipment, exposure to unprotected heights and loud noises. Responsibilities ESSENTIAL FUNCTIONS Provides comprehensive diagnostic, preventative, and therapeutic health care services to patients either on the Mobile Van, or the Outpatient Clinic. Direct Patient Care Services Interviews, assesses, and evaluates patients to determine their physical condition and type of medical services needed, and orders initial diagnostic studies appropriate for the patient’s medical condition. Performs a basic physical assessment using techniques of observation, inspection, auscultation, percussion, palpation and use of diagnostic instruments. Discriminates between normal and abnormal findings to recognize early stages of serious physical, emotional, and mental problems. Consults and collaborates with other health care professionals at Tuba City to establish health care plans and to review patient progress and response to therapy, including the use of written referrals to Community Health Nurses, specialty clinics, etc. Adheres to medical regimens to stabilize chronically ill persons and adjusts regimens as clinically appropriate. Documents visits in the Electronic Medical Record of patients including updating problem lists, medications, etc. Makes decisions concerning medical care needs of patients consulting with physicians as appropriate. Works collaboratively with physicians in the management of complex medical problems as appropriate. Provides health care and preventive services to healthy individuals including immunizations, guidance in nutrition, common illnesses, accidents, substance abuse, exercise, and depression screening. Educates patients and family members in ways to maintain or improve their health status. Carries out treatments and procedures as required. Observes and documents changes in patient’s condition, reactions to medications and treatments. Responsible for prescribing medications including computation of doses. Performs other duties as assigned. Administrative Tasks Participates in planning, instituting, evaluating, and revising program plans, assists in determining conditions, resources, and policies essential to delivery of health care services. Participates in performance improvement initiatives which may involve chart reviews. Keeps abreast of the latest medical knowledge via CME as required by respective licensing agency. Participates in hospital committees as assigned by supervisor. Participates in a teaching capacity in the development of other health care workers. Is responsible for electronic health records data entry pertinent to patient care. Ensure proper PPE is always worn while on duty including but not limited to, face mask, gloves, gown, isolation gown, NIOSH-approved N95 filtering facepiece respirator or higher, if available), and eye or face shield. Complete all donning and doffing tasks in a safe acceptable method and discard of used PPE accordingly. (see CDC website for most current updates) Complete task training for all routine cleaning and decontamination processes for all surfaces contaminated by a communicable disease to ensure a high level of patient, visitor, employee, and external customer satisfaction. Completes all patient care documentation in the electronic health record entries accurately and in real-time. Participates in departmental workflow and or testing teams as related to electronic health record or other project initiatives Performs other duties as assigned.
