Location: Lexington, KY 

Schedule: Monday-Friday 8:00am-5:00pm 


As a BCN Hospice Team member, you will be a part of a team providing care and support to patients and their families who no longer receive treatments to cure their terminal illness, but instead choose care to improve their comfort and quality of life. Our service allows our team members to focus on one patient at a time providing care where the patient calls home. While this often is a house or apartment, we also provide care at nursing homes, assisted living facilities, and hospitals. If a patient’s care needs are too complicated for home care or require a higher level of attention from nurses and other experts, we can provide care at one of our Hospice Care Centers.

About the role:

The Hospice Flex RN is a diverse role providing support to Hospice RN Case Managers to ensure patient needs are met and high-quality care is maintained in various home care settings, such as private residences, skilled nursing facilities, personal care homes, or wherever the patient calls home. The Hospice Flex RN is dynamic depending on a regional office’s most urgent needs and can involve providing care to specific caseloads, completing visits for multiple case managers, assisting with new admissions and managing referrals, and participating in on-call rotations to ensure continuous patient care. A Registered Nurse in this position must possess strong assessment skills and a thorough understanding of disease processes, as well as the ability to manage symptoms proactively. Excellent communication skills are essential for interacting with patients, families, and interdisciplinary team members. The Hospice Flex RN must be able to work autonomously, handle various patient care scenarios with professionalism, and provide empathetic and compassionate care in emotionally challenging situations. Requirements

  • A graduate of an accredited school of registered nursing. 
  • Currently licensed as a Registered Nurse through the Kentucky Board of Nursing or participating COMPACT state Board of Nursing. 
  • One year of general nursing experience is preferred. New nursing graduates will be considered. 
  • Skilled in nursing practice and able to maintain professionalism in moments of emotional stress. 
  • Provide care in a compassionate nature while serving a diverse population respectfully and professionally.  
  • Complies with accepted professional and ethical standards and principles. 
  • Understands and supports the hospice philosophy and principles of death and dying. 
  • Possesses and maintains current CPR certification. 
  • Have satisfactory references from nursing schools, previous (or current) employers and/or professional peers. 
  • Have excellent observation, good nursing judgment and communication skills.  Ability to provide accurate and timely documentation. 
  • Self-directed to work with minimal supervision and within an interdisciplinary team 
  • Flexible and cooperative in fulfilling role obligation to meet the needs of patients and BCN.  
  • Team members will be expected to comply with Bluegrass Care Navigators (BCN) Employee Health, infectious disease, and vaccination policies. 
  • Must be a licensed driver with an automobile in good working order, that is insured in accordance with state and/or organization requirements, with personal auto liability insurance of $100,000/$300,000 in bodily injury coverage.
