Shelby Skilled Nursing and Rehabilitation is a 75 - bed skilled nursing facility located in the heart of Sidney, Ohio who is seeking a Licensed Practical Nurse (LPN) to join our team and provide quality care to our residents in a nursing home setting. The successful candidate will have experience working with geriatrics and long-term care, as well as a passion for providing compassionate and attentive care.

 

The Licensed Practical Nurse (LPN) provides direct nursing care to residents or patients under the supervision of a Registered Nurse or physician. The Licensed Practical Nurse (LPN) plays a vital role in promoting and restoring patient health by following care plans, administering medications and treatments, monitoring patient conditions, and offering emotional support to patients and their families.

 

Night Shift Licensed Practical Nurse (LPN): 6:00 pm - 6:30 pm  shift differential to base pay

Responsibilities For Licensed Practical Nurse (LPN)

  • Deliver compassionate, safe, and effective nursing care based on individualized care plans.
  • Administer medications and treatments as prescribed by the physician.
  • Monitor patient/resident conditions and report changes to the supervising nurse or physician.
  • Document all care and observations accurately and timely in accordance with regulations.
  • Assist with wound care, catheter care, tube feedings, and other clinical procedures. Supervise nursing assistants and ensure care standards are upheld.
  • Participate in care planning, team meetings, and shift reports.
  • Support residents’ dignity and well-being through person-centered care.
