Job Description Summary

The Licensed Practical/Vocational Nurse is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes. 

Essential Job Functions/Responsibilities: 

  • Provides direct patient care as defined in the state Nurse Practice Act. 
  • Implements plan of care initiated by the registered nurse. 
  • Provides accurate and timely documentation consistent with the plan of care. 
  • Assesses and provides patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care. 
  • Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinical Manager. 
  • Uses and/or prepares equipment and supplies effectively and efficiently adhering to aseptic technique. 
  • Participates in personal and professional growth and development. 
  • Performs other duties as assigned by the registered nurse.

Job Qualifications:

  • Graduate of an accredited practical nurse or vocational nursing program. 
  • Has three (3) years nursing experience. Community/home health or medical/surgical experience is preferred. 
  • Currently licensed as a licensed practical nurse or licensed vocational nurse in the state. 
  • Possess and maintain valid CPR certification. 
  • Complies with accepted professional standards and practice. 
  • Demonstrates good verbal and written communication, and organization skills. 
  • Must possess a valid California Driver’s License with automobile that is in good working order and insured in accordance with the organization requirements. 

Physical Requirements:

  • The ability to sit occasionally for 1-3 hours.  
  • The ability to frequently stand for 3-6 hours.  
  • The ability to occasionally drive/ operate foot control for 1-3 hours.  
  • The ability to walk intermittently on level and or unlevel surface for less than 1 hour.  
  • The ability to intermittently bend or climb for less than 1 hour.  
  • The ability to occasionally twist, kneel, crouch and crawl for 1-3 hours.  
  • The ability to lift 50 pounds frequently for 3-6 hours.  
  • The ability to lift greater than 50 pounds occasionally for 1-3 hours.  
  • The ability to carry up to 50 pounds frequently for 3-6 hours.  
  • The ability to carry greater than 50 pounds occasionally for 1-3 hours.  
  • The ability to push up to 50 pounds frequently for 3-6 hours. 
  • The ability to push greater than 50 pounds occasionally for 1-3 hours.
