About the Role
We’re seeking a dedicated and empathetic Hospital Liaison RN to join our hospice team.
In this pivotal role, you’ll serve as the vital connection between hospitals and hospice care—helping patients and families experience a smooth, dignified transition to end-of-life care.
As a Hospital Liaison, you’ll collaborate closely with hospital staff, healthcare providers, and the Blue Ridge Care team to ensure compassionate, timely, and coordinated support from hospital to home or inpatient hospice.
Schedule: 32–40 hours per week
Location: Winchester, VA
Includes some weekend coverage and flexibility for hospital program support.
How You’ll Make a Difference
Serve as the bridge between hospitals and hospice—facilitating seamless transitions of care.
Provide education, guidance, and emotional support to patients and families making hospice decisions.
Collaborate with physicians, case managers, and social workers to identify and coordinate patient referrals.
Complete hospice admissions within the hospital setting and ensure documentation accuracy in both Blue Ridge Care and hospital EMRs.
Conduct supportive visits for existing hospice patients admitted to hospitals and communicate updates to care teams.
Advocate for patient comfort by initiating nursing interventions that relieve pain and other symptoms of life-limiting illness.
Provide formal and informal education to healthcare professionals about hospice philosophy, benefits, and symptom management.
Participate in rotating holiday and weekend coverage to ensure continuous, compassionate service delivery.
What You’ll Bring
Current Registered Nurse (RN) license in Virginia; BSN preferred.
Strong communication and collaboration skills with hospital and interdisciplinary teams.
Proficiency in electronic medical records (EMR) and computer systems.
Ability to travel throughout assigned hospital and community territory.
A heart for patient advocacy, education, and compassionate service.
