Compensation: Base Points Plus Structure, Guaranteed Base, Uncapped Potential, $70,000 - $85,000+ commensurate with experience, processed bi-weekly.

 

Shift Type: Full-time, Monday - Friday, 8 AM to 4 PM (flexible), on-call rotations.

Summary:

We are looking for compassionate Registered Nurses (RNs) to work one-on-one with our patients in their home environment to establish and oversee all aspects of case-managing patient’s care. Case management involves creating a Plan of Care for patients and following physician orders that lead to outcomes of increased patient independence and satisfaction. The RN for this position enjoys working with a team of supportive clinicians as well as being disciplined to ensure the highest level of care.

 

Responsibilities:

  • Assumes responsibility for assigned patient’s care from admission through discharge by effectively executing patient’s Plan of Care.

  • Collaborates with the patient, family/caregiver(s), ordering physicians, other care team members, and community resources.

  • Submit accurate, complete, timely, meaningful, and required documentation, including progress and clinical notes.

  • Participate in case conferences with other clinicians by reviewing services, clinical documentation, and discussing pertinent patient care issues and goals.

  • Participate in on-call rotation as reasonably assigned.

 

Qualifications:

  • Minimum of 2 years of nursing experience with a preference for 2+ years in an acute or a post-acute care setting.

  • Active State Registered Nurse licensure is required.

  • Proof of current CPR certification and valid Driver’s license.
