JOB SUMMARY:

Home Matters Caregiving is looking for Case Manager Caregivers with a passion for providing outstanding client care and relationship building. Initially, this role is focused on direct caregiving for clients. Once a Case Manager has learned Home Matters systems and processes, a Case Manager can become involved with introducing new caregivers to clients, helping with onboarding and training new caregivers, managing multiple clients, assessments, scheduling, and recruiting.

Our caregivers perform the most essential task in our industry which is providing a high level of care for our clients. Home Matters Caregiving wants to be the most valuable partner to our caregivers and clients in a competitive landscape. We provide initial and ongoing training and won the Home Care Plus Best In Class Traing award in 2023.

QUALIFICATIONS:

Must have at least 3 years of caregiving experience with the senior population
Must have sufficient communication and language skills to effectively perform duties
Associates or Bachelors Degree

RESPONSIBILITIES:

· Provide outstanding care to new and existing clients. This includes helping with Assisted Daily Living (ADLs) task such as:

Household assistance: food preparation, monitoring home safety, laundering, light housekeeping
Personal care assistance: hygienic care, mobility assistance, medication assistance, companionship
Transportation for errands or appointments

REQUIREMENTS:

· Schedule: Consistent and flexible shifts

WORKING ENVIRONMENT:

· Wherever we are services clients in their place of living. Mostly private homes or condos but sometimes careing for clients living at facilitities.

· Job Type: Full-time

The Case Manager is primarilty focused on being a caregiver and being responsible for executing the care plan for assigned clients. The number of clients could range based on location and hours of services. Client Management Provide hands on caregiver support to one or multiple clients. Compliance and Documentation Maintain accurate and timely client and caregiver records in accordance with agency policy and regulatory requirements. Document assessments, communications, and service changes in the agency’s system (e.g., WellSky, ClearCare). Assist with audits, incident reporting, and quality improvement initiatives. Collaboration Work closely with intake, scheduling, and billing departments to ensure smooth coordination of services. Communicate effectively with Home Matters management team and family members involved in client support. Quality Assurance Communicate changing needs as client(s) conditions evolve Keep up with assigned training needs and requirements. Identify opportunities for service improvement and client engagement. Qualifications: High school diploma required; Associate or Bachelor’s degree or certification in home care is a plus (CNA, STNA, CDP, LPN, RN, etc...)
