•Manage client caregiver schedule from Thursday to Sunday
•Answer phones in a friendly, professional attitude.
•Ensure client and caregiver needs are being met
•Work with caregivers and team to ensure all shifts completed including back up caregiving as needed.
•Be knowledgeable of company Policies and Procedures.

•Be knowledgeable of services provided by our agency to manage prospects.
•Manage caregiver/client schedules for all clients utilizing Wellsky software (training provided)
•Send daily report of activity and follow up needed.
•Assist with managing caregiving team on daily basis.

•Perform other duties as instructed by the Administrator.

 

Home Matters Caregiving specializes in providing In-Home care for the senior population so they may continue to enjoy the comfort and safety of their own home. We are looking for a team member with a strong work ethic, problem solving ability and above all...compassion for caregiving and helping others. Must have a minimum of 5 years of caregiving experience and prior management experience preferred.

The Case Manager is primarilty focused on being a caregiver and being responsible for executing the care plan for assigned clients. The number of clients could range based on location and hours of services. Client Management Provide hands on caregiver support to one or multiple clients. Compliance and Documentation Maintain accurate and timely client and caregiver records in accordance with agency policy and regulatory requirements. Document assessments, communications, and service changes in the agency’s system (e.g., WellSky, ClearCare). Assist with audits, incident reporting, and quality improvement initiatives. Collaboration Work closely with intake, scheduling, and billing departments to ensure smooth coordination of services. Communicate effectively with Home Matters management team and family members involved in client support. Quality Assurance Communicate changing needs as client(s) conditions evolve Keep up with assigned training needs and requirements.
