About the position: The Registered Nurse (RN) Case Manager coordinates all aspects of the patient’s home care. You will interact with other medical disciplines as needed & provide professional care to the patient. You will also perform the admission visit to the patient in the home & determine the patient’s eligibility for home care services & develop the plan of care to be followed. Qualifications:

Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing. Current state license as a Registered Nurse, Current state Driver’s License. One-year experience as a Registered Nurse, two preferred. Proof of current CPR and Hepatitis consent/declination. Reliable transportation with valid and current auto liability insurance.

Assess vital signs including, pain scale, fatigue and distress at each visit. Reconcile patient home medication list in the EMR at each visit. 6.Follow OSHA regulations regarding safe handling of chemotherapy and hazardous drugs including using proper PPE at all times with proper disposal of hazardous drugs Required Education and Experience Graduate of an accredited nursing program 1-3 years of related experience Required License/Certifications Vailed RN license for the State of Texas CHEMO/BIOE Therapy certification; BLS within 30 days of hire (ONS Fundamentals of Chemotherapy and Immunotherapy Administration within 12 months of hire for Outpatient Infusion Clinics)
