Bonus Potential Summary/Objective The Clinical Coordinator provides leadership for the unit accountability and responsibility for clinical operations ensuring that efficient and effective nursing care is provided to individuals and families in the department. The Clinical Coordinator makes decisions independently and in collaboration with the Director to promote the attainment of hospital goals in accordance with its mission. Maintains personal and staff awareness of current developments, evidence based practice and trends in nursing research with a focus on the specific patient population in which they are employed. Maintains required documentation of patient care and unit activities in accordance with standards, and ensures quality of care is maintained through Performance Improvement activities. Supervises activities of department(s) and promotes achievement of departmental/hospital objectives and ensures staff accountability by managing staff performance through rewards, recognition, coaching, counseling, and disciplinary action, in collaboration with the Director. Displays sound fiscal judgment and control of department as evidenced by achieving budget targets, including but not limited to, hour/stat and cost/stat. Behavioral Health Care Manager - Residency Clinic - Days

Bonus Potential Summary/Objective The behavioral health care manager is a core member of the collaborative care team, including the patient’s medical provider and psychiatric consultant, as well as the larger primary care team or medical team. The behavioral health care manager is responsible for supporting and coordinating the mental and physical health care of patients on an assigned patient caseload with the patient’s medical provider and, when appropriate, other mental health providers. Coordinate and support integrated behavioral and physical health care for assigned patients by collaborating closely with the medical provider, psychiatric consultant, and other treating professionals to ensure comprehensive, team-based treatment planning and follow-up. Conduct evidence-based screenings, assessments, and clinical evaluations for common mental health and substance use disorders; engage patients in care, facilitate follow-up, and assist with differential considerations within scope of practice. Facilitate referrals and care transitions to specialty behavioral health, substance use treatment, social services, or community resources as clinically indicated, and develop relapse prevention and self-management plans for patients achieving treatment goals or transitioning from the caseload. Required Education and Experience Master’s degree or higher in Social Work, Counseling, or Psychology from an accredited institution. Minimum 1–3 years of clinical experience providing behavioral health assessment and treatment.
