Assess resident needs, develop care plans, administer nursing care, evaluate nursing care, and supervise CNAs and other personnel in the delivery of nursing care. Responsibilities of the Registered Nurse (RN) Care Manager: Identify and participate in process improvement initiatives that improve the customer experience, enhance workflow, and/or improve the work environment. Complete assessments of residents at admission and discharge. Supervise nursing staff assigned and allocate duties to LPNs and nursing assistants, as directed. Provide leadership and direction to the Certified Nursing Assistants and Nurses assigned to the unit. Ensure that routine nursing care is provided to meet the daily nursing care needs of each resident. Make daily rounds to monitor resident care and status of residents. Implement established nursing policies and procedures, educating nursing support staff according to facility guidelines. Review charting on a weekly basis for accuracy and completeness. Maintain a safe working environment, complying with infection control and body mechanic procedures. Respond to need for staffing shortages by arranging for replacement staff. Track infection control, exposure control, and resident care trends to provide proactive improvement. Other special projects and duties, as assigned. Requirements of the Registered Nurse (RN) Care Manager: Current Registered Nurse (RN) license in NY. Bachelor’s degree in nursing from an accredited college or university, preferred. 2 years of supervisory experience in a hospital or skilled nursing facility. Essential Duties & Responsibilities of Licensed Practical Nurse (LPN): Assess and document resident progress in the medical record. Monitor turning and positioning of residents. Obtain changes in physician’s orders as indicated by resident’s progress or lack of progress Communicate pertinent information about resident condition and progress to charge nurses, supervisors and Director of Nursing, as indicated. Maintain clean, orderly treatment cart and restock treatment supplies. Follow Infection Control policies and procedures. Administer medications and treatments accurately as ordered by the physician and according to all facility policies and procedures. Document pressure sore status weekly or more often as condition changes. Assist in monitoring the inventory of medications, medical supplies and equipment to be sure adequate supplies of skin care products are on hand to meet the needs of residents. Participate in educational programs for residents and their family members relative to skin care when a resident is being discharged to home. Participate in Quality Improvement activities, as assigned. Job Requirements of Licensed Practical Nurse (LPN): Licensed Practical Nurse current state license in New York. Assist with promoting a compassionate physical and psychosocial environment for the residents. Responsibilities of Certified Nursing Assistant (CNA): Provide personal care (i.e., grooming, bathing, dressing, oral care, etc.) of residents daily and as needed. Change bed linen and maintain appropriate grooming supplies in resident room on a regular basis. Ambulate and transfer residents, utilizing appropriate assistive devices and body mechanics. Record resident vital signs, weights and measurements, intake and output, food consumption, and other information, as assigned. Routinely turn and position residents, as necessary. Observe residents carefully and report changes in condition to Charge Nurse. Participate in collecting data needed for the accurate completion of the MDS and the plan of care. Provide input to the nurses for development of the plan of care. Provide adequate hydration to residents at regular intervals each day according to specific resident needs. Prepare residents for each meal and transport to dining room (unless a bed patient) making sure that food tray is accessible, self-help devices are available (if needed), and assistance is provided to those residents who cannot feed themselves. Keep an accurate record of food intake and output, as directed. Be alert for proper ventilation, temperature, light and noise control. Take TPR (temperature, pulse, & rate), blood pressure, weighing of residents, etc., as directed. Requirements/Qualifications of Certified Nursing Assistant (CNA): Current Nursing Assistant certification from New York. Current/active CPR certification, preferred. At least one (1) year Skilled Nursing experience preferred. Effective verbal and written English communication skills. Highest level of professionalism with the ability to maintain confidentiality.
