Acute Care Pediatric Advanced Practice Provider

$60 / HOUR
Who We Are Imagine Pediatrics is a tech enabled, pediatrician led medical group reimagining care for children with special health care needs. We deliver 24/7 virtual first and in home medical, behavioral, and social care, working alongside families, providers, and health plans to break down barriers to quality care. We do not replace existing care teams; we enhance them, providing an extra layer of support with compassion, creativity, and an unwavering commitment to children with medical complexity. The primary location for this role is remote, travel is expected to be up to 10%, and the expected schedule is shift work (3-4 12-hour shifts per week) of days, nights, weekends, and holidays. Active TX, or LA or MO licensure is required. What You’ll Do In this role, you will provide virtual urgent care services to Imagine Pediatrics members and their caregivers as part of our innovative care model. You will help us in meeting members where they are by providing care to our members virtually, responding to inbound urgent care needs from members and their care teams, and triaging decisions to dispatch our mobile field team of allied health personnel to initiate blended virtual and in-home urgent care visits. You will: You will work in a radically different model of healthcare. Expect collaboration, shared decision making, and partnership across clinical and non-clinical care team members. Imagine Pediatrics is a team sport. Foster trusting relationships with individuals, their families and caregivers, and communities, providing integrated primary, behavioral, and urgent care for kids. Go above and beyond to connect with people in a non-judgmental, respectful and empathic manner, to meet their needs, and to provide feedback to the system as a whole as we strive to do better every day. You may at times work with an Acute Care Pediatrician during Mobile Integrated Care visits. You may virtually meet with patients, obtain an appropriate history, assist allied health personnel in a focused physical exam, order appropriate testing and treatment, and provide appropriate follow-up with requesting and primary providers. You have a stable and reliable internet connection, along with a private place to work. What You Bring & How You Qualify First and foremost , you’re passionate and committed to reimagining pediatric health care and creating a world where every child with special health care needs gets the care and support they deserve. You will need: Minimum 3 years’ experience as Pediatric PA or NP in a high-volume emergency medicine or urgent care setting strongly preferred You are Board Certified Physician Assistant National Certification Exam or Nurse Practitioner. Certified Pediatric Nurse Practitioner strongly preferred, and Certified Pediatric Nurse Practitioner - Acute Care is a plus. Experience managing medically complex pediatrics required. Bilingual Spanish strongly preferred. Virtual care experience a plus. Active TX, LA or MO licensure required, and an additional license in DC or NY, FL is preferred. Active DEA Controlled Substance Registration or eligible for application. Maintenance of professional credentialing and CME standards. What You’ll Do As a Pediatric Nurse Care Coordinator at Imagine Pediatrics, you are the primary point of contact for our families as you work to deeply know our patients through frequent virtual touchpoints and are the first line of defense when our patients are having a clinical problem. You leverage an integrated technology platform and are complimented by an entire interdisciplinary team including MDs, APPs, social workers, navigators, pharmacists, and dietitians. You will: Provide professional and friendly proactive care and triage for clinical issues. Embed a family centered care philosophy in care delivery. Demonstrate cultural competence and sensitivity as ability to work with culturally diverse populations and seek out additional resources when needed. Transition of care for ED/IP/UC care coordination with clinical providers following discharge. Perform a comprehensive assessment of a patient’s clinical, psychosocial, discharge planning and financial needs. Establishes clinical milestones and goals related to these issues. Establish rapport and a relationship with the patient and family in order to understand their needs and expectations and to assist them in setting realistic and mutual goals. Integrate an awareness of cultural factors in the patient/family interview process and elicit clinically relevant cultural information. In conjunction with the physician, the patient and interdisciplinary team, establishes a comprehensive plan of care to appropriately address clinical milestones. What You'll Do: As an Acute Care Registered Nurse on Imagine’s acute care team, you will be part of a team that is focused on delivering acute virtual care to patients in markets nationwide. In the role of an Acute Care Registered Nurse, you will: Provide professional and friendly proactive care for our patients. Utilize standard triage protocols to accurately assess and provide a disposition for clinical issues using our platform.
