Assist with patient care tasks and coordinate dining services including meal tray delivery, feeding assistance, and documentation of meal intake. Report any changes in a resident’s condition to the nurse supervisor and keep them informed of factors that interfere with being able to perform work as assigned. Maintain confidentiality of protected health information, including verbal, written and electronic communications. Responsibilities include: Direct the daily activities of the CNAs and ensure policies and procedures are followed by nursing personnel. Participate in admissions, discharges and transfers of residents as required. Assess resident’s conditions, prepare, and administer medications per physician orders. Initiate, review, and update care plans. Report any changes in a resident’s condition to the nurse supervisor and keep them informed of factors that interfere with being able to perform work as assigned. Perform other assigned duties given by clinical or administrative leadership. Maintain confidentiality of protected health information, including verbal, written and electronic communications.
