Participate in admissions, discharges and transfers of residents as required. Assess resident’s conditions, prepare, and administer medications per physician orders. Initiate, review, and update care plans. Report any changes in a resident’s condition to the nurse supervisor and keep them informed of factors that interfere with being able to perform work as assigned.
