Lead with Heart - Nurturing Care Coordination

Supervise your caseload, nurturing the growth and development of LPNs and Home Health Aides, as we cultivate a harmonious team of caregivers. In your role as a facilitator of care coordination, we celebrate the unity that arises when PRN staff steps in, ensuring seamless support for your patients.

Qualifications

Active RN License in the state of service
One (1+) Year of Clinical Experience; Hospital and/or Home Care experience preferred
Current CPR Certification
Valid Driver’s License
