As a triage clinical guide (nurse) you’ll be responsible for providing telephonic advice to members when they call Devoted for clinical needs. Bilingual Cardiometabolic Nurse Practitioner

Job Description This position represents an amazing opportunity for a nurse practitioner (APRN) with a big heart and entrepreneurial spirit who is passionate about chronic disease management. In this role you will be working in a next generation virtual cardiometabolic clinic that dramatically expands access to medications for America's most vulnerable seniors. The clinic focuses on optimizing medication adherence for Devoted Health members with hypertension, hyperlipidemia and diabetes who are at risk of running out of their medications. You will serve as the prescriber in our medication renewal service that we call “Medications on Time” and will also work closely with our clinical pharmacy guide time that comprises over 90 pharmacy technicians who reach out and support members who are approaching or have become nonadherent to their medications. You will utilize and help improve our home-grown technology and electronic health information platform to carry out virtual visits as well as managing asynchronous prescription refills for patients. The cardiometabolic clinic is one of several of Devoted's virtual specialty care programs that are designed as “micro centers of excellence” that deliver highly tailored, specialized care to patients with specific chronic conditions. Responsibilities will include: Conduct focused and thorough assessments of patients with conditions that impact cardiometabolic health including hypertension, hyperlipidemia, coronary artery disease, and diabetes who are running out of medications. We expect that 40-60%% of your clinical effort will be performing virtual visits. Formulate accurate diagnoses and develop individualized treatment plans for patients with cardiometabolic conditions, including medication management, lifestyle modifications, and monitoring recommendations. Manage a refill inbox for patients with cardiometabolic conditions in accordance with established protocols and guidelines.
