Participate in QAPI or facility assessment activities as needed, such as carrying out duties assigned as part of a performance improvement committee Facilitates, serves, attends, or participates in various committees of the facility as appointed. Ensures delivery of compassionate quality care and nursing supervision as evidenced by adequate staff coverage on the units, general cleanliness, and maintaining optimal resident functions. Collaborates with members of the interdisciplinary team, physicians, consultants, and community agencies to identify and resolve issues and improve the quality of services. Oversees nursing schedules to ensure resident needs, regulatory and budget standards are met. Participates in the recruitment, hiring and termination of nursing personnel. Performs rounds to observe residents and ensure nursing needs are being met. Conducts observations of nursing care and supervises development of in-service education to ensure nursing staff is competent in current knowledge and skills. Promotes teamwork, mutual respect, and effective communication. Monitors, assists, and implements the infection control program in accordance to current infection control guidelines to prevent the development and transmission of disease and infection. Participates in budget development for the nursing department, medical, nursing, and central supplies. Assists in the preparation of inspection surveys, including staff in-services on survey processes, and participation in mock surveys as indicated. Participates in the facility’s plan of correction response to an inspection survey and implements any follow-up required for nursing allegations. Communicates directly with residents, medical and nursing staff, family members, department heads and members of the interdisciplinary team to coordinate care and services and respond to and resolve complaints and concerns. Oversees resident incidents and concerns daily to identify any unusual occurrences and reports them promptly to the Administrator and/or state agency for appropriate action. Monitors for allegations of potential abuse or neglect, or misappropriation of resident property and participates in the investigative process. Promotes safe work practices, safety rules, and accident prevention procedures to prevent employee injury and illness. May provide direct nursing care, as necessary. Acts in an administrative capacity in the absence of the Administrator. Additional Tasks Treats all residents with dignity and respect. Promotes and protects all residents’ rights. Establishes a culture of compliance by adhering to all facility policies and procedures. Complies with standards of business conduct, and state/federal regulations and guidelines. Follow appropriate safety and hygiene measures to protect residents and themselves. Maintains confidentiality of protected health information, including verbal, written, and electronic communications. Reports noncompliance with policies, procedures, regulations, or breaches of confidentiality to appropriate personnel. Reports any retaliation or discrimination to HR or compliance officer. Reports any allegations of abuse, neglect, misappropriation of property, exploitation, or mistreatment of residents to supervisor and/or administrator. Protects residents from abuse and cooperates with all investigations. Reports any occupational exposures to blood, body fluids, infectious materials, and/or hazardous chemicals in accordance with facility policy. Participates in all life safety and emergency drills and trainings. Fulfills responsibilities as assigned during implementation or activation of the facility’s emergency plan. Reports work-related injuries and illnesses immediately to supervisor. Follows established infection control policies and procedures. As a condition of employment, completes all assigned training and skills competency.

Other Key Responsibilities for Assistant Wellness Director: Perform admission and ongoing evaluations of residents quarterly, upon return from Hospital or skilled environment, or change of condition (initiate care conference) and use this information to set measurable goals and outcomes for residents that meet the physical, mental, and psychosocial needs. Obtain written orders for any necessary changes to care plan. Completes 6 hours of dementia-specific training annually to meet the needs or preferences, of cognitively impaired residents and understands current standards of care. Verification of MARS/TARS and communicate with pharmacy to assure accuracy of medication & treatment administration. Reviews occurrence reports submitted by staff members to determine if an assessment/care conference needs to be performed on an existing resident & address any personnel issues which are identified. Schedule educational sessions relating to health for all residents. Communication of all emergency policies and provision of updated information to staff. Work toward continual improvement of the overall organization. Available to work a flexible schedule including on call, weekends, and holidays when necessary. Perform other duties as assigned. Memory Care Coordinator Responsibilities include but are not limited to: · Plan and coordinate a therapeutic program which meets spiritual, social, emotional, physical, and intellectual needs of the resident · Asses resident characteristics (i.e., stages, sex, ethnic background, prior lifestyles, cognitive and functional abilities) and, in conjunction with other departments, plans and organizes program content · Monitor daily functioning of the neighborhood to ensure continuity of, and appropriate changes in the program · Assist in developing, implementing, and conducting in service training and education of care to all staff regarding memory care programs/activities working alongside the Director of Nursing and Life Enrichment Director. · Establish and maintain a sense of teamwork through effective communications, interaction, and team meetings. Develop and maintain cooperative relationships; inform and consult with staff regarding program and integrate with other services · Participate in support groups at the direction of the Life Enrichment Director · Assess the educational needs of staff regarding program and dementia-specific knowledge and works with the Life Enrichment Director to ensure appropriate education is provided · Market the program through involvement in community organizations and participates in the local Alzheimer’s and like associations · Maintain accurate and timely documentation that complies with state regulations and community policy · Work with management to develop and maintain written program objectives and procedures for implementation; method of evaluation · Serve as a role model for staff regarding care of dementia resident · In coordination with the nursing department and Director of Life Enrichment, perform a pre-admission assessment for each potential resident · Assist with the resident’s admission to ensure a smooth transition · Assist with the adjustment of the resident and family to the community; contacts weekly for the first month post-admission · Keep abreast of current research, new programs, and community resources which may benefit residents and families and makes referrals as appropriate to facilitate the resident’s use of resources, and to promote the resident’s increase level of social functioning · Assist residents in the maintenance and adequate supply of personal clothing and other personal items · Refer the resident/resident’s sponsor internal and external services that are available to the Director of Life Enrichment · Agree not to disclose assigned user ID code and password for accessing resident/facility information and promptly report suspected or known violations of such disclosure to the Executive Director Preferred Skills and Qualifications: · Must be an LPN · Experience with Alzheimer’s and other dementia individuals · Two years of previous experience in programming: including but not limited to: POC (plan of care programs, scheduling staff, coordinating meeting with POAs and families, planning activities and working with dementia residents in an assisted living environment · Background in nursing/ proving one on one care for seniors · Flexible schedule, including availability to work evenings, weekends and holidays as needed General Working Conditions: This position entails standing for long periods of time.
