Essential Duties & Responsibilities: Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs. Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine appropriate rehabilitation goals and interventions. Monitor patient progress closely and adjust treatment plans as necessary to optimize functional outcomes and promote independence. Participate actively in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills. Maintain accurate and timely documentation of initial evaluations, treatment plans, progress notes, and discharge summaries in electronic medical records systems. Provide education and counseling to patients and their families regarding the rehabilitation process, expected outcomes, and post-discharge care. We are currently seeking Wound Care Nurse Practitioners to provide wound services to patients living in long-term care facilities. WOUND NP ROLE AND RESPONSIBILITIES: Provide wound care services for long-term care patients residing in skilled nursing facilities. Prescribe appropriate medications, including controlled substances in accordance with KY Law. Order, review, and/or follow-up with treatments and diagnostic testing. Communicate with the nursing home staff on a regular and as needed to discuss changes in condition. and/or the care plan. Collaborate with the primary care physician. WOUND NP QUALIFICATIONS AND EDUCATION REQUIREMENTS Current, unrestricted NP license in the state of Kentucky. Board Certification. DEA licensure (Optional) Clinical background in adult, family, or geriatric specialties. Clinical background of wound care; including but not limited to chronic, debilitating, stalled, or infected wounds Must be comfortable with diagnosing and treating diabetic, arterial, venous, and mixed etiology wounds or Kennedy Ulcers. Must be comfortable staging pressure injuries according to the NPIAP Must be familiar with wound care best practice and advanced wound care products; collagens, alginates, foams, silvers, etc. Must have experience with wound vacs and other DME, such as pressure relieving/ reducing devices Strong preference to having wound certification or be open to getting certified Experience working in a long-term care setting. Under the supervision of the Administrator, the Director of Nursing has the authority, responsibility, and accountability for the functions, activities, and training of the nursing services staff in the facility. In the absence of the Administrator, the DON assumes responsibility for the facility. The DON is responsible for the overall management of resident care 24 hours a day, seven (7) days per week. The DON is delegated the responsibility for carrying out the assigned duties in accordance with current existing federal and state regulations and established company policies and procedures.
