PURPOSE: The Jewish Board's Community Behavioral Health treatment programs provide compassionate, high quality, evidence-based services to individuals and families in the communities we serve. POSITION OVERVIEW: The psychiatric nurse practitioner (NPP) is responsible for providing psychiatric services and care to the agency's children and adult population in community clinic practices, including clinical diagnosis, prognosis, medication management, and discharge planning. The NPP functions as an integral part of the interdisciplinary team environment approach of the Agency. The NPP may be asked to cover for other NPPs and/or assist with administration of long acting medication when nurse unavailable. KEY ESSENTIAL FUNCTIONS: Provide psychiatric assessment, clarification of clinical diagnosis, recommend treatment options and provide client education on mental health issues and strategies for recovery and review patient's progress and recommend discharge and aftercare planning. Attend and provide feedback in case conferences. Prescribe psychotropic medication as appropriate for client care. Collaborate with other appropriate staff, i.e., nurses relative to individual, group and family therapy. Assist with urgent crisis situations by conducting evaluations, interventions, and/or consultation to the clinical team. PURPOSE: The Mishkon Division provides supports and services in a healthy and safe environment for individuals with intellectual/ developmental disabilities (IDD) living in residential programs and in community settings. POSITION OVERVIEW: The Director of Nursing (DON) is a key member of the agency’s leadership team and is responsible for the overall clinical and operational management of the Nursing Department. This role ensures the delivery of safe, high‑quality, person-centered nursing services across OPWDD and DOH certified ICF programs, and provides strategic direction that supports optimal health outcomes, regulatory compliance, and organizational excellence. Psychiatric Nurse Practitioner- Residency Program

Our year-long residency teaches Psychiatric Nurse Practitioners how to become clinical leaders at the agency and in the field of mental health. The Psychiatric Nurse Practitioner Residency includes: Enhanced supervision and support, the heart of our NPP Residency Program. Residents spend the brunt of their time in precepted clinical experiences, where they receive dedicated supervision from seasoned NPPs and psychiatrists who provide individualized supervision throughout the day, including evenings. Residents also work in specialty rotations, where they gain experience providing care to a diversity of clients, with regular supervision and support. In all clinical experiences, residents gradually build their caseloads to avoid the “sink or swim” situation that newly graduated Psychiatric Nurse Practitioner Residents typically encounter. Our Psychiatric Nurse Practitioner Residency Program encourages leadership and the ability to tackle larger issues related to mental healthcare. Residents take part in weekly didactic sessions to review and discuss stimulating readings, guidelines, case studies, and other learning materials to supplement their clinical development as Psychiatric Nurse Practitioners. Residents also participate in regular Quality Improvement (QI) experiences, identifying and addressing larger trends impacting mental health care at the agency and beyond. These weekly QI and didactics sessions also provide residents additional supervision and peer support, greatly decreasing the isolation and stress of being a new Psychiatric Nurse Practitioner. To provide you with diverse clinical experiences, the residency will include different settings and populations. Residents must be willing and able to travel to different sites throughout our agency, typically 1 day a week on-site. For example, residents may work at our programs in Riverdale, Bronx, and Midwood, Brooklyn. As part of their clinical and leadership development, we expect residents to embrace key roles in leading special projects, like providing therapy and/or meds-only treatment. Upon completion of the year-long Residency, Residency Graduates continue to work at the agency for at least a year as full-time Psychiatric Nurse Practitioners, with salaries increasing accordingly. This way, Residency Graduates further develop the relationships formed during their Residency year with their clients, Residency Preceptors, therapists, and program directors. Family Nurse Practitioner - Part time

SIGN-ON BONUS INCLUDED POSITION OVERVIEW: The Jewish Board strives to provide person-centered care that considers clients’ physical and mental well-being. The Family Nurse Practitioner’s role will be to evaluate clients in our Youth and Families Division for medical conditions and provide treatment according to established best practices; impart education around healthy living and disease prevention guided by a trauma informed approach that also incorporates harm reduction and motivational interviewing; and collaborate with mental health clinicians to effect the best health outcomes for the populations we serve. Clinical duties will be in the Supervised Independent Living (SILP) for young adults currently in foster care in the Bronx, and two group residences: Kaplan House in Manhattan, and Cedar Knolls House. KEY ESSENTIAL FUNCTIONS: Work scheduled hours at two locations: Kaplan House and Cedar Knolls House. SILP youth will be seen in one of these group residences. Most work will be in-person, but some sessions may be provided via telehealth. Provide comprehensive primary care services, including detailed histories, physical exams and diagnoses. Incorporate emphasis on preventative care and medical screenings based on established guidance. Order and interpret diagnostic tests, using results to formulate treatment. Treat acute and chronic conditions, with appropriately scheduled follow-up and monitoring. Make referrals to subspecialist care when indicated, then incorporate subspecialist recommendations into comprehensive approach. Prescribe medication in accordance with best practices. Consider range of treatment modalities and interventions that could potentially improve clients’ conditions. Educate clients about their conditions, as well as therapeutic and preventative measures they can use to improve their physical health. Utilize motivational interviewing techniques such as SBIRT in addressing behavioral change. Remain up-to-date with innovations in primary care and nursing practice. Understand the interplay between mental health conditions and somatic health, especially the physical side effects often caused by commonly prescribed psychiatric medications. Communicate as needed with multiple disciplines including case managers, social workers, psychiatric providers and medical subspecialists in a way that ensures a holistic understanding of the clients’ condition(s). Maintain medical records promptly and accurately, utilizing the organization’s Electronic Health Record. RN Case Manager

POSITION OVERVIEW: The RN Case Manager is a direct link between medical services and the ICF/IRA residents and staff as well as with clinical professionals outside the program. The RN Case Manager is responsible for the complete nursing care and performance of all routine medical procedures within the scope of the nursing practice. The RN Case Manager is responsible for supervising LPN’s, AMAP and Direct Support Professionals (milieu counselors) in the performance of nursing tasks and activities. The RN must possess a working knowledge of OPWDD rules and regulations and JBFCS policies and procedures. The RN must be capable of supervising, understanding and working effectively with diverse populations. KEY ESSENTIAL FUNCTIONS: Provide direct nursing care as prescribed by the physician/Nurse Practitioner, in compliance with recognized nursing standards and State and Federal regulations. Uses nursing process to make continuing assessments of the individuals with developmental disabilities total health needs. Maintain direct interaction with consumers on a regular basis, but in no case, at a frequency of less than once per week. Perform nursing assessments, by direct physical examination, of each consumer upon entry into the program quarterly (at a minimum) thereafter. Develop and implement an individualized plan for nursing service. The nursing care plan must be updated at least annually or whenever there is significant change in the consumer’s condition. Coordinate all acute and ongoing health services, as indicated by need and regulatory mandate. Monitor health and behavioral status, labs and efficacy of prescribed treatments. Ensure timely communication of findings to appropriate clinical personnel. Ensure that all medical record documentation is accurate, timely and outcome based and reflected in the nursing notes. Provide initial and ongoing, but at a minimum frequency of annually, training to staff in all nursing tasks, functions, protective and preventive health measures that include but are not limited to: Appropriate health and hygiene methodologies Control of communicable diseases and infections Detection of signs and symptoms of illness, first aid for accidents or illness and basic skills required to meet the health needs of the consumers Client specific medical/health related issues Seizure management Ensure that training in the above noted areas is given to all new hires and per diems prior to their assuming client specific work assignments. Provide training and ongoing support to direct care staff regarding medication administration, tube feeding and diabetic care utilizing the standard curriculum approved by OPWDD. Conduct annual clinical performance evaluations for unlicensed direct support staff for procedures that include but are not limited to: medication administration, tube feeding and insulin administration. Evaluations must become part of the employee’s annual performance evaluation. Maintain documentation of all training curriculums, attendance and post training testing. Must have experience and ability in working with electronic health record management, electronic care coordination and electronic health record interface with external service providers. Active participant in the Interdisciplinary Team (IDT). All other duties, as assigned. EXPERIENCE REQUIRED/LANGUAGE PREFERENCE: Experience with the Intellectually Disabled population preferred. Managerial experience preferred. Must possess excellent verbal and written communication skills. Ability to work in a team environment. Must be able to work independently and possess sound independent judgment and decision making skills. COMPUTER SKILLS REQUIRED: Proficiency in Microsoft Office Suite applications required.
