In conjunction with the Education Team, appropriately orients and trains newly hired staff. Supervises Pediatric Field Supervisor and all field RNs and LPNs. Performs all payroll functions necessary to ensure proper payment of staff. Reviews and utilizes reports to identify and correct errors on a timely basis to ensure correct billing and payroll. Conducts caregiver performance assessments during client visits as needed. Performs annual reviews of all staff with input from Field Supervisor, as appropriate. Works with Billing, Authorization, Eligibility and Billing Readiness teams to ensure accurate and timely information. Performs CTM Workflow in Homecare Homebase. Completes data set information, as assigned, regarding patient including but not limited to Transfer to Inpatient Facility, Death in Home, Resumption of Care, Discharge and Follow-Up. Performs and documents nursing service delivery related to patient admission and re-certification, if applicable, including the Plan of Care. Ensures documentation reflects individualized patient/family learning needs, developmental levels, cultural and social practices. What is Required? High School Diploma or GED Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing. 2+ years’ experience as a Registered Nurse Registered Nurse within the state with current license Ability to sit, stand, bend, lift, and move intermittently, with the capability to lift to 50 lbs - 100lbs. Incorporates patient care goals established in the Plan of Care, into nursing visits, as evidenced by documentation on skilled nursing notes. Communicates significant findings, problems and changes in condition or environment to the RN Case Manager, the physician, clinical supervisor, and/or other personnel involved with patient care. Reports unsafe conditions to the appropriate RN Case Manager, clinical supervisor, and physician, as appropriate. Implements the plan for patient safety, using the patient, family, and community resources. Assists the patient in learning appropriate self-care techniques while delivering quality patient care. Suggests revisions to the plan of care in response to identified patient care issues and notifies the RN Case Manager. Provides those services required substantial and specialized nursing skills under the direction of a Registered Nurse or physician. Collaborate with the RN Case Manager during normal business hours for advice, assistance, and instructions. Documents medication regimen and updates medication profile according to established procedures. Treats patients and caregivers in the highest and most effective manner. Implement all available actions to prevent avoidable hospitalizations and ER visits. Provides positive, supportive communication to physicians, patients, families, visitors, and agency personnel. What is Required? Graduate of an accredited Certificate, Diploma, or Associate School of Nursing Current State License as a Licensed Vocational / Practical Nurse 1 year of experience in a clinical care setting Experience in a hospice or home health environment is preferred 50% travel required Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.
