Overview We are seeking a dedicated and compassionate Registered Nurse (RN) to conduct in-home supervisory visits for EPSDT and VA clients. This role focuses on oversight, assessment, and support rather than skilled nursing tasks. The RN will manage a caseload of approximately 50–70 clients, ensuring required face-to-face visits every 60 days in the client’s home. This position requires strong organizational skills, independent work capability, and a commitment to high-quality, patient-centered care. Monitor for changes in the patient’s condition such as weight loss/gain, self-care abilities, and indicators of disease progression. Communicate with family members, physicians, case managers, and other prescribed individuals regarding the health of the patients. Provide education and training to the patient, their family, and/or the Home Health Aide. Work with patients to be done one-on-one in the patients home to provide for a comfortable environment. Collaborate with and supervise Certified Nursing Assistants and Personal Care Attendants. Perform other personal care services as necessary to meet the patient’s needs. \n $65 - $65 an hour RN Hourly Rate: $65 an hour + mileage.
