Wound Care Provider (NP/PA) | Facility-Based Rounds | Grand Rapids NIVA Health is hiring a Wound Care Provider in the Grand Rapids area to deliver advanced wound care through a facility-based rounding model. You’ll work in partnered clinical settings such as skilled nursing facilities, rehab centers, and assisted living communities, collaborating with on-site care teams to improve healing outcomes and patient quality of life. This role is a great fit for a clinician who enjoys clinical autonomy, a consistent workflow, and working alongside established facility staff. What you’ll do Provide comprehensive wound assessments and treatment plans Deliver advanced wound care services, including dressing selection, treatment protocols, and ongoing monitoring Perform debridement and other procedures within your scope and credentialing (training and support available) Coordinate care with facility nursing teams and leadership Document visits clearly and efficiently in our system Communicate recommendations and follow-up plans with the care team Work setting Partnered facilities in the Grand Rapids area (SNF, rehab, assisted living, and similar settings). Structured rounding schedule based on patient needs and facility census. Supported onboarding, clinical guidance, and operational coordination.
