Position Overview The licensed vocation nurse (LVN) is responsible for providing safe, compassionate, and focused nursing care to assigned patients with predictable health care needs. The LVN works under direct supervision of the registered nurse (RN) and assists with carrying out the patient’s plan of care in accordance with the state’s nurse practice act. Former emergency department or urgent care experience required. Essential Job Functions Provides safe, compassionate nursing care to assigned patients with predictable health care needs as directed by the registered nurse and the patient’s plan of care in accordance with state’s nurse practice act and organizational policies. Performs assigned responsibilities in a manner that promotes quality and safe patient care and excellent customer service. Completes accurate and timely documentation of patient care that reflect the plan of care. Completes focused patient assessments (defined as an appraisal of the patient’s status or current situation) in accordance with state’s nurse practice act and organizational policies. Administers medications in accordance with organization’s policies. Report data to assist in the identification of problems and formulation of goals/outcomes and patient-centered plans of care in collaboration with patients, their families, and the interdisciplinary health care team. Communicates and collaborates with patients, their families, and the interdisciplinary health care team to assist in the planning, delivery, and coordination of patient-centered care. Maintains a clean and safe work environment. Adheres to required infection control, isolation procedures, and aseptic techniques in the delivery of patient care Maintains patient privacy and confidentiality of information and records at all times Other Job Functions Attend staff meetings or other company sponsored or mandated meetings as required Perform additional duties as assigned Basic Qualifications Education Required: High School diploma or G.E.D and graduated from an approved licensed practical nurse/vocational nurse (LPN/LVN). Experience Required: Two years of relevant experience as a Licensed Vocational Nurse (LVN) or as a Licensed Practical Nurse (LPN) in an Emergency Department or similar experience; preferred Certification/Licenses/Registration Required: Current license as a Licensed Vocational Nurse (LVN) or as a Licensed Practical Nurse (LPN) by the state in which you practice Current Basic Life Support (BLS) & Advanced Cardiac Life Support (ACLS), Pediatric Advanced Life Support (PALS) certification from the American Heart Association, required Pedagogy IV Certification Course must be completed within 30 days of hire Position requires fluency in English; written and oral communication

LVN - Days (Full-Time) | Stone River Recovery Center | San Antonio, Texas About the Job: The Licensed Practical/Vocational Nurse works under the direction of a qualified professional nurse. Participates in the planning, implementation, and evaluation of nursing care, within the objectives, standards, and policies of the department and within the parameters of the licensing board. Communicates and works collaboratively with members of the multidisciplinary team i.e., medical staff, administration, clinicians, dietitians, and milieu personnel. Takes reasonable care and precautions to ensure high client safety standards and a therapeutic environment exist. Roles and Responsibilities: · Provides direct and individualized nursing care to an assigned group of clients under the supervision of a registered professional nurse, while always maintaining the assurance of client safety and following unit and facility policies and procedures. Complies with all company safety policies and practices. · Assists in relevant health and psychiatric teaching for client and family. Serves as a role model for clients and peers. · Shares responsibility for the planning, implementation, and evaluation of nursing care plans and nursing care. · Effectively communicates with the medical staff, receives verbal/written orders, and takes care to minimize abbreviation or other errors. Participates in team conferences, sharing knowledge, information, and ideas among team members. · Prepares, administers, controls and accurately records medications in accordance with the policy regarding the administration of medications by a licensed practical nurse, with thorough documentation regarding administration effect and/or adverse effects. · Observes, documents, and communicates with/reports to the R.N. in charge any clinical, physical, unit or personnel, or other issues which could alter client care. Reports status changes and concerns to registered nurses and doctors. Communicates with all departments as needed to assure total quality client care is delivered. · Consults, follows, and documents with sufficient detail nursing care according to facility policy and procedures. Records client symptoms, responses, and progress concisely, accurately, and completely. · Safely and accurately performs vital signs, electrocardiograms, phlebotomy/labs, and diabetic/glucose monitoring. Interprets results and takes appropriate nursing action. · Provides coverage in emergency services as needed. · Graduation from an approved Licensed Practical/Vocational Nursing Program required. · One or more years' nursing experience within clinical area of service provided by facility preferred. · Knowledge of and experience with psychotropic medications and medication administration procedures. · Ability to apply nursing standards and practice within a recovery setting. · Valid current practical/vocational nursing license from the state or from a nurse compact state, where applicable. · CPR and de-escalation certification required (training available upon hire and offered by facility). · First aid may be required based on state or facility requirements.
