Manage patient access to healthcare and connect individuals with community resources for medication administration, self-care techniques, and disease management. Conduct medication reviews to identify potential side effects and drug interactions. Develop and administer mental health assessments to screen for depression and evaluate mood, behavior, and cognitive function. Accurately document patient medical history—including illnesses, medications, allergies, surgeries, and family history—using electronic health records. Successful completion of the NCLEX-PN (National Council Licensure Examination for Practical Nurses). Knowledge of medical terminology and clinical documentation standards. Experience with phlebotomy, vaccine administration, and point-of-care testing. Excellent communication and organizational skills. Experience working in a clinical setting with diverse cultural dynamics among staff and patients. Proven ability to manage sensitive information ethically and responsibly. Pay Range The typical pay range for this role is: $21.10 - $43.78 This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.
