How You Will Make a Difference

As the Certified Nurse Aide, you will:

Work in collaboration with the RN to fulfill the defined patient-specific Certified Nurse Assistant care plan
Collaborate with the care team as an extension of nursing or therapy services to ensure all patient needs are fully addressed
Deliver hands-on personal care and supportive services to assist with safe transfers and ambulation
Thoroughly document care delivery daily in our Electronic Medical Record system
Contribute to a culture of caring through individual accountability and teamwork

Experience to Deliver on our Mission:

High school diploma or equivalent preferred
Hospice certification required for State-specific certified Certified Nurse Assistantcertification
State licensure or a competency evaluation program, valid state driver’s license and auto liability insurance
One year experience as a Home Health or Hospice Certified Nurse Assistantor Nursing Assistant in a hospital, nursing home, or home health agency. Home health experience preferred
